Don't have any picture/video since I was so shocked when it happened. Buuuut;
I was building a giant china wall with scout towers on. So in the middle off night(far away from a house with a bed in it) I stand in one of the towers looking out over the rest off the wall(to see if it was safe from the inside from mobs). Turn around to look out over the gate. See a creeper coming up the ladder that is in the tower, he goes SSSSSSssssss *boom*, sends me flying upwards and outwards, which ended with me falling a bit over 60 y-grades and loosing all the items ,_,

Biggest WTF moment?
On my second map, I made my first nether portal, went through, explored a bit, and came back to find... nothing. I couldn't find my portal! Although I could swear it was the same place and I could even hear the sounds it made. I later realized that a stream of lava decided to cover up my portal from me.

One day I decided it was time to start a new file. I started playing a little and I got myself a stone house for some protection for the nightfall. An hour in, my computer overheated and shut-off. Once I rebooted, to my dismay, I found out that immediate shut-off had erased all progress (I had saved at least once).
A while later, I started a new map. After a week or so, I explored a ways away, until of a sudden, I saw a cobble structure in the distance. I was really confused, since I know cobble doesn't occur naturally above ground. As I got closer, I realized it was my old house from the deleted file. THAT was a big OMG WTF moment for me.

Well my biggest WTF moment doesn't actually have anything to do with minecraft servers being messed up or glitching. But I had finished my house and I decided to get off for the night while my friend.. the admin of the server was still playing. I come back the next day and I'm just kind of digging around and BOOM! Game lags out and i get kicked off. I get right back on and I see a giant crater where my house used to be. Evidently my friend dug under my floor and put tnt underneath and then put dirt above it... such a ***** move hahah!
That's happened to me before. I made a castle type structure on one of my earliest files. Deleted it to make room for a new file and a day or so later on a new file I'm wandering around at night and I see torch light, and git really confused. Turned out it was the castle, even had the hole in the wall from the creeper attack.zehydra said:

I once took 80,000 blocks of TNT to the wall of this one SMP RP server by hollowing the inside of it, and my friend who helped me accidentally struck on of the blocks before we were finished and a whole 1/3 of the wall around the city was destroyed >:O

Logging on the SMP server I was playing on to find someone had dug flawlessly into my hidden underground/water bunker and destroyed everything, despite LWC being on.
Followed their path up to the surface and the entire map was covered in lava towers and fire, and people were flying around with diamond swords killing everyone.Also there were cobblestone penises pretty much everywhere.

I was building a shaft downwards to search for ores, and the shat was inside my house, protected by steel doors. When I later climbed out the shaft, I went to my furnace to melt some steel. As I put the steel in however, I hear a sound from behind.

I was mining in a cave, and then the night came, so, since I was too far away from the shelter, I just said "fuck it" and kept mining deeper
so I kept on, found some diamonds, gold, iron ore, and as I was going back up, the game crashed and I closed it
so when I re started it, I loaded the game and it spawned me inside a stone block which started tearing away my health
so I pulled the pick axe and started picking the shit out of every direction until I had a space big enough to stay without losing health, and so I started mining my way up again (thankfully, I was carrying my craft table with me, you know, just-in-case) and so I went up
I had like 1 1/2 hearts left and when I came up a creeper was waiting for me
thankfully (again) I had mined my way up a fucking mountain from the inside, so when I faced the creeper, I just slapped it and he fell all the way down the cliff
