The worst thing I can think of.. corrupt savegame... really bad when you've almost completed the game.. made me snap a gamesdisk in 2 at one point, think it was Fallout2

That god damn bear in Condemned 2..... seriously I was like "WTF I have to fight a bear?!?!?" Completely random. Also in Silent hill 1 when you are in the dark with your weak ass flashlight and the radio starts going off, totally creepy.

For me it is water areas in some games. It all started way back when I played Banjo-Kazooie on the N64 and when I reached the Treasure Trove Cove level and jumped off the dock into the water and all I hear is "dun dun dun dun dun dun dun dun" and a FRIGGIN SHARK appears out of nowhere and tries to eat me. I can do water levels now-a-days but I have never truly recovered.

That made for some hilarious screenshots where EVERY unit onscreen (and off) were desperately shooting a translucent figure as a nuke hung onscreen...
