Anything that involved going into a realistic underwater area of a game that had critters in it. Like in Fallout 3, those freakin' Mirelurks (while cool looking) scared the crap out of me!
The worst game ever was this one on the PSX that was an underwater treasure hunting game. You had to fight a Giant Eel in the first level.... *shiver* UHG! Needless to say, I want to play that game now that I'm older.
Treasures of the Deep for the PS1? I loved that game! What was worse was level 3 when you had to fight some monster... thing that can take a mini nuke and still not die. That was such an awesome game.

Psi reavers. Also the 'defend your position' sections in Half-Life 2, because I know I'm not going to get to move on with the story for a looong time.

In the old Resident Evil Games. I was wandering around in a hallway. There were three zombies. I had just survived an encounter with another monster. I had my shotgun. The zombies were closing in on me, getting close together. I could kill them all with my shotgun, which was currently equipped with, in one single shot. The placement was perfect. They got closer and closer and when they finally lunged at me, I pulled the trigger(or pressed the button...) to blow them all away as planned. However....

*click*

I was out of ammo.

THAT"S THE ONE! HA~! It was truly an awesome idea, but, like i said, created my weird hydrophobia in gaming.
Seriously? Man, now I've GOTTA get a copy. ^w^
Thanks for the game title. You are made of awesome!

Crimson Heads + bad controll system of old Resident Evil games + limited saves = terror at its purest form.

My worst generic fear is when you get into a fight that you know you're going to lose for the sole reason that you are the wrong class/character etc. It really pisses me off, for example, when I get killed by a shit spy as a sniper, or a shit soldier as a pyro (both from TF2) just because they are my anti-class.

Scariest moment of any game, however, was in Eternal Darkness (on the GameCube), when I went to inspect a bath to see if there was anything in it. From what I saw before inspecting it, it looked empty, but when I inspected it the bath was full of blood and I saw my own character lying in it, dead. Man... that game properly fucks with your mind.

Bad check-pointing, playing games where death forces you to retread twenty minutes of gameplay give me the shivers.
As for a particular enemy, Deathclaws in Fallout 3 if you don't see them untill they're up close, ~gulp~ nothing can survive that.

I can =p

But they cut straight through armour, I can take them out if I see them when they're still a ways off, but if they get the jump on me it's likely I'll be evicerated.
Obviously you did not get my "console command" humor =/
~silly console gamer feels silly~ I'm guessing it's some sort of god-mode?

In Gears 2, hearing a very low pitched "BOOM!" while standing in open ground.
Oh my, I can identify with that. Those nasty beasts have being the undoing of many a Horde session.

Otherwise, the distinct "beepbeepbeep" of a landmine in Fallout 3, specifically the one you haven't seen until you stumble near it. Also, thanks to Broken Steel, rounding the corner into the face of a Super Mutant Overlord.

It when you get past a really hard part in the game, forget to save, and then die a moment later by something simple and then it take you ages to get past the one hard bit again

Im always worried that one day I will never get past that one stage again! Will I be doomed to repeat that one level forever!!!

It's ok, sometimes I just assume people memorize console commands =p
