Opening a closet door in L4D, normal mode. Turned on a flashlight, startled a witch, who was literally a foot away from me. I had luckily decided to pick up a shotgun (the better one) and emptied my entire clip into her. She died, I didn't take any damage. Still that first moment of realization was amazingly terrifying.

Also, playing Nazi Zombies in COD5 and having all the lights off, and my friend, who was sitting behind me, grabs my shoulders. I spun around in my chair and almost punched him, purely out of reflex.

in Doom 3, the first Pinky Demon that shows up.
it looks in the window and cant get through so it heads for the door which it cant break down. i sighed in relief and assumed i would have to fight it later. suddenly it falls through the roof and roars in my face and backs me into a corner.
i was scared and dead.

Last year I was feeling a bit reminescent and hooked Shining in the Darkness on my Genesis. Anyway I got as far as where you get all three characters and I was crazy deep in this dungeon and I have like a really horrible sense of direction to begin with right. Well, my warrior dude gets wiped out followed by the healer on my team leaving my whimpy little mage. I was like oh crap oh crap oh crap and stumbled out of there, running from like 10 fights with no health or recovery items left. >.>

I was playing Oblivion not too long ago and was just skill grinding (I have a careful strategy to max out my stats and skills by grinding primaries and secondaries in a specific order). Anyway, greed got the better of me and I stuck my nose in deeper than I should have gone in this cave and ran up against a troll. I tried some tactics and realised.. oh snap I left my silver and magic gear back at the castle so I ran and ran, outside, attracking wolves now.. and a bear.. and ran, and ran.. horse was missing, ran some more. Finally I reached town and ducked through a door. (best part was when i went outside like two days later the bastards were still waiting on me)

Like many others, my first encounter with a Deathclaw.
Walking up to the Super-Duper Mart I see a...thing that killed a bunch of raiders, so I think "Hey, maybe she's friendly!"[small](for some reason, I think of Deathclaws as female)[/sub] So I walk up, see the red mark "OH SHI-" next thing I know, she's digesting me.
I now know to avoid them.

The first F.E.A.R. game when you're in the sewers and go to step down a ladder. You turn around to go down and BAM! evil little girl in your face. Then, after jumping out of your skin, you get attacked by ghosts after landing on the floor below.

I actually had to pause the game for about five minutes I was shaking so bad. (personally I'd have thrown myself off the ladder, but that's just me.)

In Left 4 Dead 2, at the end of the first level. I was running towards the helicopter and heard the tank music start up. I really thought I could make it... I swear, it felt like I was in mid-leap for the helicopter when the tank swatted me out of the way. I didn't last long after that.

Playing L4D2 online with a few friends, trying to get the Expert achievements, and ended up with just two of us barely alive during the Heavy Rain campaign, about to end the second-to-last chapter. As we limped toward the safe room, a Tank spawned on the safe room's door, and charged at us. As I died under his big, meaty fists, I shouted to Stevey "It's too late for me, save yourself!" He responded with "Screw you, I've been doing that all night!" He then proceeded to panic as he heard the screech of a Hunter, which then pounced him, and ripped off his face. Our other friend, who was already dead, could be heard cackling maniacally as it all went down.

I've had worse, a meeting with a deathclaw at the superduper mart at level 2 with only a 9mm I somehow won though losing all of my ammo too.

EDIT: damned ninja's, at least I survived the encounter.

Two moments come to mind:

Near the beginning of Doom 3, that small room with a window looking out into darkness. No obvious threats, and a recording on the desk in front of you. As soon as you move towards the desk, BAM! An imp lands on the window outside. Scared the bejesus out of me. (Though not as badly as a mate: he was playing in the dark, wearing headphones. The imp lands on the window, he shoots back in his desk chair... unfortunately, beyond the limit of the headphones cord....)

Second moment, many years ago, playing Strip Poker III on the Amiga. Just about to win the lady's final piece of clothing, and my mum says, "What on earth are you playing?!" Never knew she was so light on her feet....

I was playing Medieval 2: Total War in the Americas campaign as the Mayans allied with England against Spain. Right after taking a Spanish city I notice that England had about 5 armies sitting at its doorstep. The next turn they broke the alliance and used their cannons to break down the walls around my new city. I actually yelled oh shit, run and gave then gave the order to my troops. Last time I trust those shady English peoples.

In left4dead 2 the tank is a decent challenge on normal difficulty, but when your playing on expert with friends not to mention in a car lot, it dousen't yeild the best results.
