The End of Evangelion, seriously.
I don't think it's possible to watch Rebirth without getting completely freakedout/confused/scared/wtfowned at the same time.

The last few episodes of Berserk rather creeped me out. The whole Eclipse and the sense that everyone is royally fucked.

Except Guts, of course(and that's because we know the whole series is a flashback).

The story is built strongly on the theme of hopelessness. You get to watch the slow dissolution of sanity into paranoia and savage murders. Due to the way the story is structured (I won't elaborate because of potential spoilers) you get to watch some pretty horrific things happen to the characters you care about. Higurashi doesn't exactly handle its characters with kid gloves.

The needle in the ohagi wasn't actually a product of the hallucination or the paranoia. You have to remember, Mion comes from a yakuza family. Their sense of humor is a bit...different. Read the manga and it's pretty clear that it legitimately happened as a (horrible) prank.

And if you remember that the main charachters range from 12-16 at the most it puts it more into perspective (Higurashi)

And goddamnit I am going start having nightmares again thanks to School Days.

When they cry, or, Higurashi No Naku Koro Ni is pretty freaky. Also, Neon Genesis Evangelion with the scene with Asuka and the classical music... I can never bring myself to listen to classical music after watching that episode...

I Like Akiba Girls. For something more on topic though, I think that Alucard can get pretty awsomely freaky at times. Interesting thing though, no anime truly freaks me out, not even the consept of the Haruhi thing just get Agent 47 (or Kratos) after all, it would be easy to kill her as she is not omnipresent so you wouldn't have to worry about her expecting it. Plus existentialism is stupid, or at least this part is:

I would go with higorashi or elfen lied, although elfen lied has a good story once you get past the first episode.

Very good choice. Freaky on so many levels. Elfin Lied didn't really freak me out, I just thought it was bad ass :p Haven't seen School Days yet but I want to. Evangelion wasn't freaky, just annoying and overrated. For my pick though I'll have to go with "Betterman". I've never seen a show make someone eating so creepy o_O

The idea behind Haruhi was chilling, but the creepiest ive seen is definatly soul eater... Once the wavelength of insanity comes in it gets creepy, not to mention the Kishin wich is basically an embodyment of chaos itself.

In the original run of Fullmetal Alchemist, there was a quite disturbing episode. It was the 6th episode, it had "Scar" splatter the chimera-girl all over the wall. I would say the entire Fullmetal Achemist is twisted.

Nah. It nicely built off them and resolved a lot of the plot threads left in the series. Not to mention had some cool battles and a great song to end the world to.

Without the movie, what would be be left with? Some kind of bizarre series of still frames and no plot resolution whatsoever, except maybe Shinji finally getting Self Esteem while the rest of the world congratulates him? Thank God that NEVER happened.

I was thinking that at first, but it isn't really scary. For creepy, I'd go with Blood+. Vampire children get very scary...

The original Blood: The Last Vampire movie was lovely, because the vampires in that show are so twisted and distorting looking (put the sparklepires from Twilight up against one of those and see how long they last!). I have yet to watch Higurashi, but I know enough about it to know it's MESSED UP. I will probably love it. XD I have the entire series of Elfen Lied too, but again, haven't gotten around to it.

But I adore Boogiepop Phantom anime series, and the Boogiepop universe in general. Manga-wise, I was really creeped out by Uzumaki. Seriously, how do you make something as everyday as a spiral that freaky? The author's earlier work, Tomie, was a interesting read too.

I'd have to agree with you, the gore can be a bit much but really it's not that scary, well maybe the start of episode 1.
