Died when he got between me and a group of mirelurk kings I'd just launched a mini nuke at. Like any good companion would, I looted his corpse and replaced him with a mutant.

HK-47, Jolee, Carth, Canderous and sometimes Bastila from KOTOR1; Atton and T3-M4 from KOTOR2; Garrus, Wrex, Tali and Joker (if he counts) from Mass Effect; Goris from Fallout 2; and of course the Weighted Companion Cube.
Ashley and Kaiden from Mass Effect (also Liara) Wrex was a bit of a pain, only because he is so big he can obliterate your view of Sheppard (Esp. if you are a female)

Dogmeat from Fallout 1.
He was such a help in the mid section of the game, and I was heartbroken when I got to a point where no matter how many times I reloaded the muties killed him.

Johnson and Arby from Halo. They ruled. I know Johnson was the black guy from Predator. Look. Look at how much I care.
I liked all my buddies from Freedom Fighters. I tended to waste Medkits healing them when I could have just recruited more dudes.
Somebody mentioned Agro from SotC. I have to agree there. Once I got used to how he actually had his own AI and just sort of let him steer most of the time, my complaints with the controls became a lot fewer. Unlike certain other horse characters in games, Agro felt like he had personality.

The entirety of your squad from Republic Commando, especially Scorch as performed by the ever-wonderful Raphael Sbarge.
On the note of Sbarge and Star Wars, most of the party in Knights of the Old Republic with Sbarge's Carth Onasi, Bastila Shan (I, at least, warmed to her somewhere along the way to Manaan) and Mission Vao.
I guess the british squadmates in both CoD2 and CoD4 will have to make the list as well, with Price+McGregor in the former and Price+Gaz in the latter.

Elika saved my hide in Prince of Persia a lot. The winner must be Fawkes. A Super Mutant with a gatling-laser. Damn it's awesome sight.
