Rurouni Kenshin comes in a close second, but more OT: I really don't like the intro for Kenshin that much.
Been watching Ergo Proxy recently and the opening and closing sequences are really good. It's mostly the music, which has a kind of strange juxtaposition with the show's sci-fi/cyberpunk aesthetic, but also fits really well with its melancholy, contemplative and existential themes.
Can't really think of any that are especially bad. I've got tired of a few because of seeing them over and over in long anime marathons (Death Note, Initial D), but they weren't necessarily bad.

I thought the opening to the first season of Bakuman (haven't seen the other seasons yet) was a bit too slow and mushy for that series.

The problem with the Vandread 2nd Stage opening ('Justice') isn't that it's bad as such, more that it's so much worse than the utterly kickass First Stage opening ('Trust').PedroSteckecilo said:

Not a huge fan of the ending song for this anime but I've heard much worse. I tend to skip the intros to most animes, if possible some of them just take far too long.
