Joe Kucan is the man, but he isn't a voice actor. For voice acting, I would have to go with David Hayter. The worst would have to be the guy who did the voice for Mass Effect Sheppard guy.
Hard choice really since i don't care what most people sound like. I'd nitpick Dynasty Warriors 6 but i love that game series too much Plus if you hear that annoying voice then it drives me to them to get rid of it xD!

Dantes voice in Devil May Cry 4 doesn't suit him at all. It should be deeper.
Also, every voice in Oblivion is bloody annoying. They were probably okay at first, but they're used over and over so much that I can't even remember a time when they didn't fill me with an urge to murder.
I usualy only notice good or bad voice acting in animes. I find some voices annoying in games, yes, but I never really notice the voice acting.

The Spy on TF2 has a neato voice. And G-man.
I'd say marcus phoenix from GoW and g-man have the best voices and the worst would be Eli Vance from hl2 and Master chief in halo2
p.s. I have hl2 and halo 2 in spanish so the english version might be better, btw halo 2 was translated into south american spanish which I dislike due to the fact that I'm spanish

Worst, I have to say it though i love the game too death, is master chief from halo. Im mean, its well written dialogue, but his voice is more smarmy and too-cool than dante's from DMC!

Awful awful game, but Steve Blum lends his voice to Vincent Valentine very well. I've always been a fan of that voice actor. Beyond that, Auron for Final Fantasy X. His voice was great. Can you tell I'm an RPG nerd yet?
Worst? Altair from freaking Assassin's Creed. I don't care. The fact that he had practically no accent and a monotone 'tough-guy' voice sucked when you had convincing accents on every other damn character. Bad decision from whoever cast him.
