We all know The Doctor is destined to die. But we also know they won't kill off ye olde Doctor due to the massive uproar that would ensue. Moffat would be lynched killing off a TV legend in the space of 2 series of him lead writing. So it begs the question ..... how the hell is he going to pull off a decent ending without it looking cheap and cheesy as fuck.
Well to be honest, cheese is what Doctor Who is made of. As mentioned earlier in this very thread, Last of the Time Lords can be easily compared to The Big Bang in terms of their tacky 'Power of Love' endings.
My prediction is that the next episode is going to be less about the Doc's death, and more about what River does about it. Changing a fixed period of time is tough to do and can have horrific consequences (see Father's Day), but can be maintained if you are willing enough to cause a rupture in time (see 10's power trip in The Waters of Mars - the woman commits suicide to soften the damage). Will River stop the Doctor from dying (hint: probably), and what consequences will that have?
...in fact, I think that's what this season is leading up to. Silence Will Fall when River decides to wreck time and space to save him.
