Not being able to move and fire in third person shooters really creeps me out. I start to panic. That's why I don't like Resident Evil games.

Tightrope walking missions (damn you Sonic Unleashed), Regenerating Enemies (Dead Space and Resident Evil 4, you're guilty of this!
Underwater levels, more for what's usually in underwater levels.
No-one likes them, they aren't fun.

small enclosed spaces, I am almost always ranged I hate fighting in caves... nice big dungeons are awesome but winding caves just have me paranoid I"m about to be raped by big nasty goblins.

Large areas between checkpoints. And offscreen enemies in horrorgames, the last one is satan in the gamingworld

for some reason im very afraid that im going to get killed and fuck things up. like in Fable 2, when you have to go to the crucible, i dint because i was afraid i would die a lot like in the original fable arena mission.

whenever I'm going on a steath mission in Far Cry 2, I will have all silent weapons, slowly creeping into the enemy bases. Take out a few gaurds and keep going, my heart races when crap like that happens. But I get a heart attack when im doing said mission and a car ramms me into the wall, i swear to god my heart misses about 5 beats whenever that happens.

I used to be terrified of the wilderness in Runescape. Luckily i stopped playing a long time ago but anytime i went there i would start to shake like crazy. I guess its the fact of losing all the stuff that you worked for.

In-game I'm terrified of fighting monsters underwater. I always get nervous when I have to fight that beast in Half-Life that you'll end up in the water anyway just to get the crossbow. And Deus Ex made me yell in fear when I saw one of those beasts swimming towards me wanting to chomp me to bits. I hate..HATE fighting underwater baddies. T_T *shivers in corner*

Dark corners/halls in zombie games, or really any survival games. I always manage to overlook a shadow and bam, I'm dead.

Huge open areas of water and the sound of giant insects walking around (namely the ones in Fallout 3).

In HL2, the head crabs. Or more focus the ones that already ate someone head. Just walking around and out of no where "OMYGODHELPME!". IN the middle of the night, in a dark room with a small light. FREAK ME OUT!. But the worst are those damn fast head crab zombies. I hear them and freak out and look everywhere until it to late and their some how behind me.

Couldn't of said it better myself. Beating Prototype was a serious chore for me because of those.
