The Dune Beetles in act 2 of Diablo 2. You know, the ones that die and shoot lightning everywhere? Yeah, those.
The venom lords that breathe fire in act IV are worse IMO

Jumping Geth (Mass Effect), Headcrabs (Half Life), and spiky flying turtles (Paper Mario: The Thousand Year Door)

Jumping Geth were annoying because I had a slow shooting gun, Headcrabs would always sneak up on me when I had 1-5 hp and kill me, You couldn't jump on the spiky flying turtles, and you couldn't reach them with your hammer.

Cliff racers.
Ooooh, yes, Cliff Racers were a serious pain in Morrowind, especially when you were lower level and desperately needed to rest outside of a dungeon.

But I think Pyramid Head spooked me more and was waaaaay harder to deal with in Silent Hill.

Any kind of giant insects or spiders etc. And the bloatflies from Fallout 3. Those things terrify me.

The exploders and little baby things with tentacles on their backs in Dead Space are gigantic nuisances and a pain in the butt to get rid off, especially when they come along with their bigger and stronger friends.

I'd have to go old-school and say Koopas.

You hop on one of those bastards and send the shell careening into four of his buddies down the line, starts off all well and good...

Then before you can say "WTF", that bloody shell has ricocheted off of a pipe just appearing at the edge of the screen, and bam, it takes you out before you can jump.

Or, if you DO manage to jump out of the way, that damn thing bounces off a pipe -right- behind you, and hits you just as you land!

The Combine-Zombies from half-life. 'Zombines' I suppose you'd call them.

You hear everyone say "HURR DURR YOU CAN SHOOT THE GRENADES IN THEIR HANDS!" But I can never seem to do that, so they get withing a few feet of me, explode and take half my health.

But they're not all that bad, they still make the funny sound when you set them on fire.

In Oblivion, there are the Conjurers/Necromancers/Heretics/Zealots: essentially any enemy who wears a robe and fends you off with a wall of summoned creatures while they run away. They aren't really problems for faster characters, but for a slow heavily-armoured warrior such as me they are intensely irritating.

Point in example: some mod-creating sadist made a Necromancer boss with a staff of paralysis. Cue ten minutes of chasing one guy around the entire dungeon while constantly being zapped and falling over helplessly for three seconds.

Another notable incident was the discovery of a surface camp filled with five Heretics. Everything was going well until I pretty much ran out of Fatigue, and could therefore do no damage. Hence fifteen minutes of running after these two douchebags who kept summoning Fatigue-draining little anorexia demons and throwing spells at me while they backpedalled furiously. Eventually I pushed one off a cliff and beat the other one to death in a corner, and boy was it satisfying.

Any enemy wearing impenatrable bullsh*it armor instantly earns my hate.
The dogs from Call of Duty deserve a special mention. As do any enemy using the insanely overpowered MP40...

I also hated the Super Mutants in Fallout 3, and the fast Headcrab zombies from Half Life 2. They both scare the crap out of me...

The walkers from universe at war. I dread having to kill them, because theyr'e so powerfull and take ages to kill, and after you kill one you realize the enemy has another one just behind it.

Rebel Artillery and Rebel Torpedo Frigates in Empire at War. Goddamn hellspawns... Why does the AI have to spam those so much? Why? WHY?
