I was hunting Vista's in Guild Wars 2, for people who don't know what Vista's are, they are a little cinematic camera swoop around the environment as a reward for solving a (sometimes rather complex) jumping puzzle.
Needless to say these vista's are on rather high locations. On a top of a mountain, edge of a cliff, etc.

After spending nearly an hour on trying to get to one, refusing to give up and look on youtube, I finally got it. I was on the same platform, just needed to take one more step.
Until I accidental hit the Walk-Forward button twice.
This makes your character do a little forward dodge roll. Right off of the cliff.

Smack, dead.
I have dodge-rolled off a cliff so many times. Turned off the double-tap to dodge now; works better mapped to a key, but till then ...
The worst of those was when I traited to take less falling damage - a side-effect of the trait puts out AoE damage when you take fall damage. Rolled off the cliff and survived, with a sliver of health. Only the damage hit a passive mob, making it attack and finish me off before I could stand up and heal. The icing on the embarrassment cake is that falling to your death doesn't damage armor; but being pecked to death does.

No More Heroes: Bad Girl's playing possum insta-kill. Considering all the crap Travis survived up to that point, it made no sense whatsoever. I think it was the only time I actually died in that game.

Also, trying to rob a hooker in GTA only to find out they're packin' too.

Probably every time you play dark souls you're going to get something dumb happen, example yesterday I was in burge heading down the short cut down to the Capra demon. Turned around to kill some hollow soldiers following me.
i locked onto one, the other one jumped lunged behind me my auto lock spun me 180 off the edge.

Also dmc3 second to last boss I have more trouble than any other boss in the game his two flying fish he shoots at you when you fight with Virgil always hit me its like I have no reaction time.

Also have finished dark souls newgame+++ & [dlc]
Still die to Capra demon like a noob always block his over head strike and get dogged spam rather than rolling.
I seriously don't know why I keep blocking in entry.

Borderlands 2, near the beginning. You know when Claptrap tells you to run into the electric fence? I did that. And kept doing it because he kept telling me to. And I died.

Probably one of my most memorable ones would be jumping into the well at Firelink Shrine in Dark Souls because I thought it'd be a shortcut or secret.

Every time I pass there I see a bloodstain from other players who had the same bright idea as me.

I convinced a couple of my friends to do that when I first played multiplayer with them, I laughed and laughed and laughed.

Pretty much any turn based Final Fantasy game where a boss somehow gets to attack three times in a row and each attack is an instant death...it's that, or playing something online like Call of Duty and the spotty knifing gets me killed when the person wasn't even looking in my direction.

I jumped out of a helicopter in Battlefield Vietnam, landed, started capturing a base, and had the helicopter fall down and explode on my head killing me.

Also, anytime there is something on fire and I just want to check to see if it will damage me.

And figuring out which barrels explode

I have to say my most stupid death of all had to be when I was slaughtered in Skyrim by a basket. Yes. A wicker basket. Maybe it got a letter from his cousin detailing how an Argonian ruffian had decided to place him over his boss' head and held a grudge against me.

Also lagstab and cliff deaths in Dark Souls. Always so bloody stupid.

I have one that (sort of) happened to my sister.

She was playing the Sims 2 (PlayStation 2 version) and was using the free play mode. She had a man who was more than halfway up the police force career and she had missed work once or twice now. I told her she wasn't going to make it the third time while her man was dressing into his police uniform as the police car pulls up to take him. He starts running toward the car and my sister says in a matter-of-fact tone of voice "I am not missing another day of work!"

Just then her guy gets to the car only to have it drive off without him. A second later the prompt comes up telling her that he's been fired, thus the death of his career. (It still counts)

Well I started a new game on Skyrim. I was an Orc about level 8 I was doing a bit of questing for treasure to build my house up. I got ambushed by a Sabre cat I managed to take it down and noticed 3 mud crabs near a pond, needing mud crab chitin for ornaments I ran over ignoring how much health I had and was instantly butchered by the 3 off them. Felt like a moron.

Long story short I was killed by a mud crab.

Yeah I thought that was case but apparently Phoenix Down's are for a knock out and apparently Aeris got actually killed in a cutscene.

Believe me, my girlfriend has lectured me about this like 500 times.
They say that but... knock outs are death. Taking a firaga to the face when you're at low health doesn't knock you out, it freaking kills you.

A creep getting the final hit and killing you in DOTA 2. As a noob, it's a bit more than frustrating. At least I'm not going online yet. Another DOTA death involves going off by yourself and trying to get horde of creeps to attack a tower. A bot comes around and kills me. I guess I can blame this on my noobiness in the MOBA genre.

And then there's the classic 'forgot to take an Estus Flask' one. So many deaths due to thinking I'll survive. It's bad when a Hollow ends up ending you. It's worse when poison damage bites you in the ass. It goes like this:

"Let's play through the Painted World without any Flasks!" *Gets through most of the way and does right before Priscilla.* Oh well.

Magmaw in WoW when cata had just come out. Every single week i'd run through him and fall to my death in the lava behind him on my rogue at LEAST once. This went on for 5 weeks.

Oh I thought this would be about pointless, forced cinematic deaths in games a la Thane or Aerith.

Anyway my best story is probably when last year I began playing World of Warcraft again for a brief time. I was flying over Northrend because I'm a cheapskate and discovered, to my dismay, that flying over Wintergrasp is not allowed any more. My character fell the equivalent of thousands of feet and the corpse lay in an irretrievable location.

Mine happened in the good old Mojave wasteland

Being new to fallout I really didnt know about the whole rad thing at all so right once I hit lv.7 I ventured out aimlessly with only my shotgun and a machete (I forgot to bring ammo for the shotgun anyway) I eventually ran into a really high rad area and while running around going Jason on them well I died from the rads ... wow I am a fuck-up

Sine Mora, get hit and your power ups fly out of your ship, this is nothing new in oldschool shmups, however what the oldies did right and Sine Mora doesn't is the power ups bounce off the edge of the screen, the enemies stop firing and you get a couple of seconds of invulnerably to get you a chance of collecting them, because without your powerups the game becomes really difficult.

So with Sine Mora if you get hit when your ship is at the top or bottom of the screen half your pwer ups vanish instantly, plus you have to quickly race to collect what you can, no good if your in a boss fight and you have to quickly try to navigate a mass of bullets to do so which more often then not results in getting hit a second time.

This wouldn't be half as bad if I could just not bother collecting the power ups and sticking with a peashooter, but since you are on a timer which on harder difficulties is very stingy makes bosses impossible to beat with a peashooter.

Honestly what a shitty and broken game it is.

Ah, the good old days. The only time I ever have ladder issues these days is the occasional jaunt into Left 4 Dead, for some reason the ladders in games that use Source are vehemently opposed to me climbing them.
