Yeah, small guns is definately the most useful all-round.

Melee starts to get handy around level 13-14, which is when I usually start dumping skill points into it after maxing out the four you previously said.

Repair and Medicine are both infinitely useful. Take anything that isn't stapled to the floor and sell it to Moira Brown.

The best skills for your standard playthrough are: Small Guns, Repair, Lockpick and Science. The best weapon you will obtain along the way would be the Hunting Rifle (make good use of VATS). Regarding your SPECIAL:

You don't need a whole lot of Strength if you keep in mind to loot only what you need
You don't need Charisma if you're playing a standard build (plus, if you don't mind sliding your IRL Karma Meter sliding all the way to Evil, you can save scum and pass all Speech checks, up to you tho).

As for Perks, the only important one I can recall off the top of my head is the one that allows you to sneak at full speed. VATS stealth kills is the quickest way to dewal with most threats. Also, grenades immediately explode upon contact if thrown in VATS.

Also, try reverse pickpocketing enemies for the lulz. For even more lulz, kill large creatures with mines/explosives. If you're lucky, you'll see them blast off into the stratosphere, never to return.

EDIT:
My character was a melee monster. Meaning high Endurance and Strength. It's difficult at the beginning, but once you get a Dartgun and/or a surplus of landmines (you'll have a lot if you disarm them). Plus, Deathclaw Gauntlet. With the right setup, you get a 100% chance to get a critical hit that ignores armor. Even those Conclave/Mirelurk/Deathclaw/Yao Guai bastards die in one hit from a successful sneak.

Start off by getting Repair, small guns and medicine, I cannot stress how useful that is

Don't bother with unarmed skill, I've never once been unarmed and neither will

Get the medicine bobblehead in the vault when you start off and you won't regret it

Keep all your ammo and medicine on your person, it doesn't weight anything and it has killed my brother quite a few times and he doesn't obey that rule

Keep a hunting riffle and a combat shotgun on you at all times

Don't bother with stat boosting perks, by the time you reach level 20 (or better level 30 if you GOTY edition) you will be so powerful anyway you will have wasted the perks

Be a good player, there are more places for you to stay and it gives you much better rewards in quests

Don't sell ammo or medicine and keep hold of armour that can repair yours. You will never know when you need your armour repaired

Finally save whenever you exit a fight, you won't lose nearly as much time as I have if you keep saving

Hope that helps my friend (this is all coming from a guy who has finished every single quest, found every location and finished all the expansion packs)

Look up the bobblehead locations on the net (easy to find) there's one for each skill (+10 to that skill) and one for each SPECIAL stat (+1 to each).

The best weapons in the game by far (stat wise, assuming you've increased small guns) are:
Xuanlong rifle (chinese assault rifle)
Lincolns repeater(hunting rifle that is 100% accurate and fires .44 rounds)
The Terrible shotgun(combat shotgun)
Ol'Painless(hunting rifle)

They are all better than the guns that can be used to repair them, by quite a large margin. Making a sneaking headshot with the lincoln repeater will fell almost any enemy, even deathclaws will be almost dead if you pull it off, although they are hard to sneak up on. Infact sneaking attacks in general are just a good idea because you get an automatic critical hit, which means you kill them faster and save bullets.

Nuka-grenades are devestating, much more so than frag grenades.

Killing deathclaws can be killed VERY easily by employing the dart gun. It will cripple their legs meaning that there is no way they can catch you. It's literally like shooting fish in a barrel!

You can easily raise every skill to 100, especially if you collect the bobble heads and make sure your intelligence starts high and that you collect the intellgence bobblehead from rivet city early on. (unless you have the broken steel add-on, if you leave the bobble heads until you become level 30 you can actually get 10 for every SPECIAL stat if you choose the almost perfect perk when you reach 30. This is more a matter of personal preference than whats best)

If you're planning on doing a lot of exploring then take the education perk - this means each skill book raises your skill by two as opposed to just one. You'll be surpised just how many skill books are out there if you look!

And finally....don't fire the MIRV underground. Unless you want to see your character get vaporised :)

Lockpicking is important also do not underestimate power weapons skill. I wouldnt suggest sneaking and melee weapons as your main loadout.

Make sure you get the energy bobblehead while in the Enclave base. Its one of two bobbleheads that can permanently remain uncollected.

Also, sing along with some of the game's kick ass music.

Steal isn't worth anything. Ignore it unless you don't actually want to shoot people.

Look for medical supplies. Especially the one IN YOUR ROOM WHEN YOU TURN NINETEEN!!!

Dump points into lockpick and small guns.

This is very good advice.

It means you will get more skill points every time you level up.

Just don't talk to anyone in the science lab if you want to experience the main quests in the intended order.
Might I add, set your game to very easy until you've received the Int. Bobble Head.
You get less XP for kils in Very Easy.

Once obtained, set back to appropriate level.
Listen to this man, he speaks the truth.

I can't believe I never thought of that.

But honestly, just get the hunting rifle put points into lock pick, sneak, repair, and whatever type of weaponry makes you smile, There are awesome guns for every type.
