Loki from Marvel Comics. He always has a plan, even when he is "helping" the heroes, and convincing them he has their best interest at heart, and seems and acts completely sincerely and honestly, he is always working a shady angle, even though you can't see it coming.

Scorpius. Even if he was overused by the fourth season, he was still a creepy, manipulative vicious bad-ass who drove John to insanity, and part of his head rotated out on a regular basis.

I'd have to go with Lavos from Chrono Trigger. Without a single line of dialogue it manages to project a shadow of dread over the story. This thing is a parasite that feeds on worlds and when it's had it's fill, it rends them asunder to cast it's spawn across the void. Really, how many villains have the destruction of a planet as a mere phase in their lifecycle?

So far I've seen some good ones in here. Brick Top's a good one, as is Pennywise the Clown. I'm super stoked someone mentioned Stringer Bell just because I LOVE The Wire but he's not crazy enough to be considered a top for me.

Tyki Mikk From D. Gray-man is pretty creepy, as is The Millennium Earl.

The twins from Black Lagoon as hold a special place in my heart for how sick they are.

I'm sure I have a massive list of them but I'm drawing a blank atm >.<

Benjamin Linus from Lost back when he was still a full fledged baddie is among my favorites. Ishamael/Moridin from the Wheel of Time series is another one up there. The most recent version of The Master (John Simm) from Doctor Who is also a favorite. Finally Guy of Gisborne (Richard Armitage) and the Sheriff of Nottingham (Kieth Allen) from the recent BBC series Robin Hood are two more that I get a kick from.
