Once when playing stalker SOC in the underground I saw the dancing invisible blood suckers for the first time... shudder...
actually come to think of it, most of mine have to do with those appearing and scaring me.
Towards the beginning of Bioshock, I was looking around a then empty room, go to a cabinet to search it, then the room fills with fog and I hit crouch. Turn around, and I'm looking right up into the face a crazy doctor splicer just standing behind me that wasn't there 2 seconds ago...
Also playing through Left 4 Dead on expert, looking through dark empty rooms in a hallway, when out of frickin nowhere, the tank music starts and it spawns right next to me, smacking me into a wall and killing me before I even realized it was there.

The "guy stuck in the wall that yells while he spits his guts at you" in Dead Space. They just don't die quickly enough.
Fighting the Leviathan in the old Divine game. That bugger was the size of a whole freaking country...
Not really "WTF IS That", but "WTF WAS That?". My first time hearing the AWP in Counter-Strike. My stereo was turned up to 11, and I ducked for cover as my CT avatar cartwheeled into a window and ragdoll-killed my teammate.

On topic... meeting any one of those random encounters in Pokemon (see: legendaries in Crystal). After seeing little rats, pigeons, and the occasional bull, seeing a large, majestic dog-bear would make you jump in fright.
Especially at six years of age.
