Well I hate spoilers, but it seems Aerith (yes, her name is actually Aerith) has already been spoiled all over this forum. So yeah, her death hit me pretty hard. But... SPOILERS SPOILERS SPOILERS - the hardest fictional death of all time for me was in an anime called Clannad. The second series, to be precise: Clannad After Story. One of the main characters died, and the following events kept me bawling for the last 15 episodes of the show. And I don't cry easily. That show was freaking amazing.

Games:
Father from Fallout 3, sure it was a crappy Bethesda gimmick moment (like the death of Uriel Septim VII in Oblivion), but dang, he was so nice, and so caring.

The party member you must choose to kill (not the one you have a possibility of averting) from Mass Effect, their death scenes are very touching and truly make you question your choice. Also the death of Saren if you manage to convince him to kill himself, he thanks you before he pulls the trigger.

A death scene that doesn't really qualify as a scene but, in the Darkness, when you go to the Otherworld, there is a horse there whose mouth has been cut off and a basket full of apples in front of him, he's meant to represent famine, I put it out of its misery every time.

Corbec especially, but Bragg and Caffran were pretty depressing too.

I knew he was going to die but i didn't realise how dragged out it was going to be.

But for some reason it affected me.

as for me well, I get pretty pissed when dogmeat dies (in fallout 3) so much that I'll go back a level to keep him alive, also I don't have broken steel so that new perk wont do
when gandalf dies in moria I literally whimperd, of course that lovely deus ex machina that brings him back helped that

Man, I'd forgotten about that. Best/saddest ending to one of the best FPSs I've ever played.

Anyway:From games:
Kaiden or Ashely. They were my two least favorite characters (although they've both since grown on me), but it was still a gut-wrenching moment.

Also, the suicides of Fai Dan and Saren.

From books:
Alistair McKeon's death at the end of At All Costs. By all accounts the most major secondary character, he'd been there from the beginning, starting as a Lieutenant Commander, dying as a Vice Admiral at the Battle of Manticore. Same goes for Andreas Venizelos a few books earlier. And you just know that Andrew Lafollet is going to die in the next book, he's escaped death too long when others, like Mattingly, died.

Henry DeTamble from the Time Traveler's Wife.
Amber and Kutner from House.
Charlie from Lost.

Also, this one time a friend convinced me that Wilson died in the House S5 finale (I had not watched it yet) and I started crying. I hated her for a week after actually watching it.
