There's this game on newgrounds... I cannot for the life of me remember what its called, but you only get 1 chance to save the world from a plague. I won't spoil the ending but when they say you get 1 chance they mean it.
All of the moments I can think of come from Mass Effect 2 and 3

losing most of your crew if you don't do the suicide mission right after your crew is taken,
any character death during said suicide mission, Thane's death, Legion's death, Mordin's
death, the final chat Shepard has with a romanced character in the final rush, and last but
not least Anderson's death
No video for this one (don't want to look for any). Basically, imagine you're Shepard going round the galaxy with your ship and crew and kicking ass. Towards the end your ship is destroyed with your crew inside and you only watch this from the surface. Later on it turns out that the big bad arranged it because he was actually a traitor who you knew. Your crew found out and they were disposed of. But they tried a last desperate effort just for you.
*In MGS3, the Speech The End gives before he dies.
Its a fairly unique scenario in that he wasn't really developed much in terms of exposition, but after a 3 hour boss fight (yeah I was overly cautious) I felt bad when it ended. Then he delivers his speech I felt both happy and sad. I'm sad that the only genuinely good Cobra member is dead but happy because he died exactly how he wanted.
Then he explodes and his false teeth go flying... DAMN YOU KOJIMA!
Same game, the final hours or so of the game, confronting the boss, her speech, the exposition given after the fact and finally the very last moment of the game where it looks like everyone should be celebrating but Snake doesn't and then the scene that follows (real tear jerker).
Its a bit hamfisted in its delivery, but I find it really easy to get into stories like that and see past the cheese.
*In MGS2 I always get gutted to Otacons breakdown at "that" point.
*MGS 4 has a few sequences, but they are all piled at the end and I don't think I can even passively mention them without spoiling a major sequence.
Not quite so much anymore, but the end of Act 1 of the Darkness and the ending itself was definitely in my top moments of absolute heartbreak. Still sad upon repeats, but I kinda stopped playing it because it's ruining my memory of my first runthrough of it. So many tears.
This. The Darkness made me fall in love with a character and then made me pay for it. A friend of mine laughed through that particular part on his first playthrough of the the game and it made me reassess his humanity.

I agree with this. but I find the exact moment after the fight where
You the player, have to make Big Boss execute the Boss, his mentor, and the person whose death leads to the events of every Metal Gear game in that moment, the saddest part of the whole game
When Mono, Agro, and baby Wander are in the garden, the camera pans out following the hawk and the only piece of uplifting music in the whole game starts to play.
Mass Effect 3's Extended Cut with the synthesis ending. EDI narrating about the Reapers helping us rebuild, the DAMNED sad music that nearly brought me to tears and my entire crew, whom I'd grown to love, standing together and honoring Shepard's sacrifice. Damn it! That really played my heart strings like a violinist in Halo.

The creatures we were sent here to tame are untamable, and the colonies we were sent here to reclaim have proven to be stronger than anticipated. The writing of Starcraft 2 doesn't hold a candle to Brood War.
OT: The part I really remember having my heart tugged at was the three times the song of healing was used in Majora's mask. Darmani, Mikau, and the mummy guy's scenes were so beautiful. I also thought the original ending to Fallout 3 was very well done and very emotional (didn't quite fit the game, but that's another story).
You struggle to get up the mountain, you "die", come back to fly practically endlessly in a beautiful landscape, and when you finally land you simply walk into the light. The music also played a major part. It doesn't matter how many times I play it or even listen to "Apotheosis," I tear up EVERY time.
