Amnesia the Dark Descent kind of made me stressed as in "Oh f!*% what the hell in all the name of scary is going to happen next! Let me go around this corner and-HOLY S#@$ AND PISS IN MY PANTS! TURN OFF COMPUTER, TURN OFF, MAKE THE SCARIES GO AWA-A-A-AYYYY!" Yeah, that game scared the crap outta me.
I can't think of any particular game... but the top 3 annoying things in games for me:
QUICKTIME EVENTS! IF YOU MAKE A QUICKTIME EVENT I WILL
HUNT YOU DOWN AND RIP YOUR FACE OFF AFTER DEVOURING YOUR LOWER INTESTINE AND JABBING OUT YOUR EYES!
*Cough* erm... carrying on
Mandatory time limits that result in death are devastating, I love time limits that propose themselves as 'Time attack' modes, or if you get a bonus for finishng a level quickly, but you should not die or be severely penalized over time limits, it kills any and all exploration :/
Also, apparent glitches, the game crushing ones, the glitches where you go through a platform or die instantly with full health, the game breakers that SHOULD have been fixed BEFORE you started selling a game on the mass market. If these kinds of glitches are frequent, and still left in, SOMEONE who made it has no business in making games to be sold.
The mission "Three Leaf Clover", aka the bank robbery mission, in GTA IV. The mission is easy at first, but it's super long and if you die anywhere in the mission, you have to start all the way back to driving to the bank in the first place because there are no checkpoints. Several other missions in the game also have this problem.
Also, any escort missions. Especially ones where the escortee runs away for whatever reason and you have to go back and get them. Thinking of Second Sight in particular.
Going for the obvious one, Call of Duty (from WAW onwards) its just annoying when people use the same weapon over and over...
WAW it was the MP40
MW2 it was the UMP45
Black Ops isn't as bad but the AK74u is only just used less than the start button, at least theres some more variation in BO what with there being a lot of guns
Well recently its been Assassin's Creed mutiplayer. I mean I throw down my smoke bomb why cant I stun them!. Also the Arizona Killer mission in New Vegas. I mean whats the point of popping a stealth boy if as soon as I get out the door Im detected by a damn dog! Also Dead Money....Fuck you radios!
I've saved Zelda (at the start). Completed one of those temple things. Now I have 2 flashing orbs on my map and don't know how to fucking get there (to either). Been trying for months.

Starcraft2 leaves me in a cold sweat and shakeing after just a couple games.
Im not even very high ranked or anything. Im just really bad at handling adrenaline, so as a result I tend to cower out of playing ladder and stick to custom games vs people I know.
I actually play a lot better in these games as Im more relaxed.
Damn that Asshole is a hacker, knowing what ive played and making me cry Eventualy I had to changed the controller round for him to die, How is one meant to know that!
well for me pretty much saints row 2 because it doesnt have auto save and IT IS UNSTABLE AS HECK FOR ME :"ah five hours of work and ive gotten all of my favorite cars including the armored SWAT van with the mounted minigun and .....wait whats happening? NO NONONONONONONONONONO AAAAAAAAAAUGHGHGHAFHJALISUHDSADHBSAOLUDGHALDWAD"
