I think it is... Jak II. Good god, those checkpoint races were awful. I have many others, but I think that one is the oldest.

I have yet to beat a single Final Fantasy game, more out of nostalgia than skill, and Shadow Hearts: Covenant.

Wait, you have them and never beat them? When I got them, I played until I beat them, my parents had to drag me away from the TV on Christmas morning...

Anyway, I never beat:
Second Sight (it got annoying...)
Jet Force Gemini (you are forced to rescue every fucking tribal in the entire game (or whatever those bears are that I keep blowing up...) so that means playing everything over at least twice (and you have to pick the right character for the job because there's three of them => boy, girl, dog))
Knife Edge Nosegunner (I can get to the final boss, but he is IMPOSSIBLE)
Splinter Cell (got annoying like Second Sight...)
Command and Conquer (for the N64, mostly control issues, and difficulty)
and others...

Oh and it took me a about 5 years to actually finish a game of Civ 3 (my attention span was shit).

there are a number of older console games (NES, SNES, Genesis) that I never finished, but the only "recent" game i could not finish was the first gears of war.

i could never kill that final boss. after a sad number of frustrated tries, i just gave up and watched the ending on youtube. it's one of the few games that successfully "defeated" me.

Fallout 3. I've tried, oh how I've tried, but the game crashes so much I've just decided to walk away forever.

It seemed to crash after 3 saves, autosaves counted too, so if I forgot to do a complete reboot of my computer after the 2nd save at the next save/zone I would crash the next time I went into combat...I'd pull VATS...everything would say 0%...and I'd rage...

Kane and Lynch, i got up to the first mission in Cuba where i pretty much gave up due to the bad AI constantly killing themselves by charging ocoverut of into the enemie's fire, that and the overall shittiness of the game.

Far too many of them :D probably more than I HAVE completed.

Tony Hawks ... Gran Turismo (various - except I think I DID beat the "last" race on GT2-jap waybackwhen, course that save doesn't work with my legal UK copy now) ... Loom (lost my bit of paper with spells on) ... Any of the Microprose or Maxis sim games with win-conditions EXCEPT Civ 1 ... etc... Shit sucks.

Oh and pretty much any FPS type game ever. Maybe just a symptom of ADD or something. But I did once get as far as Shub-Niggurath on Quake and simply couldn't kill it. (No-one let on about the telefragging thing)

I never beat Gruntilda at the end of Banjo-Kazooie. :(

I also never got all the stars in Mario 64.

I never beat Halo: Combat Evolved on Legendary, alone.

I'm sure there are others.

I haven't beaten any of the Devil May Cry's, except for the 4th one since I don't have it. I still haven't beaten Grand Theft Auto 3 or Shadow of Rome. I'm not sure if I've beaten Bomberman 64. I'm sure there are lots of other games in my possession that I've not beaten yet. Mainly on the PS2 and N64.

Majora's Mask, I have all of the masks but I'm stuck on the damn water temple, I mean in both OoT and MM the Water Temple sucks, in OoT the temple is a nightmare to navigate and in MM the boss is a pain to fight.

Metroid Prime. I'll admit it, Metroids freak the hell outta me. I have 2 fears: Spiders and Facehuggers, and Head Crabs and Metroids just completely freak me out. I was able to get through the game by picking them off from around corners and with power bombs (with my eyes closed), but then you get to the Impact Crater and there's unending streams of Fusion Metroids...I just couldn't handle it.

The most recent game i have never beaten would be SILENT HILL 2.

Seriously this game was getting into with my head so hard i threw my controller to the ground and turned of the tv......

Although I've managed to beaten every game I putted my hands on,Planescape Torment was the only one which I didn't manage/got the time to go around it to finish,even though the story was of epic proportions.

Silent Hill - Played 2 first, and well... SH just wasn't as good as its sequel.
MGS3 - I know everyone says its the best one, but I just don't see it. It's the only one I didn't finish, including the PSP games.
Dragon Age: Origins - I just don't have that kind of time anymore
Fallout 3 - Ditto
FF XII - just didn't like it
Mario 64 - I played Mario Galaxy first, and it was really hard to take that step back in control conventions, graphics, and sound
Ocarina of Time - Same as above, but with Twilight Princess
Battlefield: Bad Company 2 - I just don't like it. The mechanics, the story, any of the characters.
Army of Two - Sold it back after beating one level.
No More Heroes 2 - The third form of the last boss can suck one. I breezed through the entire game, and now it's gonna make me work for it? No. Screw you. You are a Wii game. I'm going to go play something fun, challenging, and devoid of an undeserved smug sense of self-satisfaction. You are not as cool as your predecessor.
Crackdown - I just don't see this game's appeal. Maybe because I played Prototype and InFamous first.
and finally...
Dead Rising - I know it's supposed to be difficult, but there's a fine line between "high difficulty level" and "broken mechanics with punitive save function".
