Picked up a harmless Sentry Gun in Portal 2, I think it was falling down due to everything around me collapsing or so. Thought I had saved it from its fall into nothingness. I gently put it back on the flo-BOOM!!
Suddenly it just exploded. I did NOT see that coming, made me spaz out a little. Cheap scare, but it worked.
On the other hand, the intentional crushing of a Sentry Gun before my eyes (along with the way I was supposed to walk on) didn't touch me at all. Huh.

Running like hell from a rage mode Rathalos in monster hunter tri. The fear he'll cheap combo you is far too great to even attempt to hit him.

OT:Call me easily spooked,but when I first got Diablo 2 when I was 10 or so,I couldn't get past the first map,because the combination of mutilated zombies,angry spine hurling devil porcupine,and creepy music made too uneasy.
So yeah,when I got through the first act and came upon the second act's final boss room and saw nothing at first,I felt confident with my awesome lighting+frost scimitar.
Then that ugly bastard Duriel charged at me at what appeared to be hypersonic speed and had his way with me.I was so panicked,I litteraly couldn't fight back properly.

Needless to say,I now pay him a visit once in a while with a level 99 character and pimped out stuff.Fucker will pay forever.

In Pokemon, when the last guy on your team is JUST about to die and you have no potions/revives left.

Pretty much any FPS, if I run into an enemy close range as a sniper, I just freak out and start firing everywhere. usually works.

And while this didn't make me panic, (it was more of an "OH SHIT" moment,
In Portal 2, when Wheatley sprigs the "suprise" on you early, during the final boss fight when Wheatley booby trapped the stalemate button, at the end when GLaDOS deletes Caroline, and at the very end, right before the turret choir.

I only had this problem when I first played it. Now I know where to find awesome revolvers early on that last until about the fascist/communist warzone level.

The sensor-mine beep (when you step on it) from Fallout 3. I start spazzing out like hell whenever I hear a noise similar to it in any game.

In BC2, I panic when either my vehicle's health reaches zero, or when I realize that my ATV is headed straight at the barrel of a tank.
Also, Nightmare House. I'm very easy to scare.

But nothing is worse than back in the days when I owned an original Playstation and I tried to load a game only for it to tell me that the save file had been corrupted.

I don't know if that counts as panic or rage. I think I felt both.

Is that the part where you get on the platform to get cake but instead there was fire? I panicked and quickly was able to put the portals in the place I needed to.

One time in Left 4 Dead we were playing expert on No Mercy, I think the second chapter of it, We were at the part after the generator. We jump out the window and the other 2 survivors died by the tank. So it's just me and someone else, we were just about to enter the safe room when a which is RIGHT IN FRONT OF THE DOOR! We jumped her and got in right before she got pissed.

I have several.

First, that moment when fighting a particularly hard boss, who is about to kill me, as I realise that my last save was 3 hours ago. Fuck you, Dead Rising 2 and your psychopaths.

Having a spider in Minecraft jump on my head from a tree, while I'm completely unawares that it's there. That shit FREAKS ME out... -.- AND their eyes glow...

Playing a fighting game online, or with a friend. I'm trouncing the guy, and they begin to make what seems to be an unavoidable comeback

Serious Sam. Whoever thought up those skeleton, horned thing is an arsehole...

Killing Floor. Last zombie of the wave, we all make our way to the trader, and it turns out that last zombie is a FleshPound...

Not a 'scary' moment... but in Batman Arkham Asylum is was terrified at the point when scarecrow attacks you for the third time and the game seems to crash and goes back to an alternate opening sequence... I restarted 3 times thinking my game had broken. :(

Fighting a big daddy in the 1st bioshock. The only times a game has sped up my heartbeat. Especially if you didn't see it and a stray bullet hits it, and you're not prepared. Those moments were the best thing in the game, gameplay wise.

Both 'push the button' moments. Especially after the first one when Wheatley takes control of the facility and opens up an elevator, then traps you in it and drops you down the shaft.
