As for games I've played with an AO/18+ rating, that I can remember after 4 days without sleep would be Doom series up until Doom 3 (which sucked big time), Wolfenstein 3D, Quake, Soldier of Fortune, Painkiller, Manhunt and Postal 2 (though the latter one is more comedic than action-packed)
Games and movies today can go fuck a donkey. Thank heavens for Stallone and his latest Rambo movie and The Expendables. They went all the way with old school, violence, blood and gore.
