In Deus Ex, freakin' Gunther freakin' Hermann and his freakin' invincibility. On easy mode, just to prove a point to a mate, I took on Gunther Hermann and he took 20 GEP gun (rocket launchers capable of destroying tank size robots)shots and he still persisted in surviving. Oh hell, I emptied every bullet in my entire inventory on him and he didn't have a scratch on him. So I had to restart the game and basically ghost every mission so I had enough bullets to kill freakin' Gunther freakin' Hermann.
Wow, really? I killed Gunther without any hassle at all. Then again, I did tell the doctor guy to stay behind, and he came up to me in Paris and told me the phrase that I could say to Gunther to blow him up immediately.
Aw well yeah, thats taking the easy way out. Really, I knew that. Hooonest. I asked the Doctor guy to go to Hong Kong, and now he may or may not find me later. I played the game before so I wanted to betray UNATCO as soon as possible and the easiest way was to kill Navarre and then later Hermann.

PS - I managed it too with a few more rockets :)

What about with the fact that in every FPS, when you look down you have no legs.

Why do none of these generic brown-haired protagonists have legs?
More and more often a game DOES feature legs which are visible, in the case of the Halo series both Halo 2 and 3 feature armored legs visible below. I seem to recall another game series that had them as well, so it's not something as rare as it once was. Of course, making a third-person shooter dodges the need for this nicely. The absence of legs is usually a holdover from the sprite-based style of gun arm animations or programmer laziness, since you have to do the same things as with the gun wielding arms a second time for the legs, you can't just use the model other players can see.

I dislike the old "vital to the plot therefore indestructible" shtick. Most recent example I can think of is CoD4, where you take over the Russian base to try and capture some spod. I thought, "hmm, while I'm up here, I'll just put a couple of rounds in that Jeep's tyres in case he tries to escape". The tyres were, of course, made from that new fangled bullet-proof rubber, and said spod proceeded to lead us a merry chase in his Jeep. Grr.

(This is also linked to the non-physics-model-compliant racing opponent mentioned above. They're taking a set route and god help you if you try and stop them!)

The magic healing properties of food.

'Cos, yeah, like, when I get a bullet to the face, all I need is to gorge myself on bread, or to shove cans of meat down my gullet and I'm good to go...

Homeopaths eat your heart out...

Also: The inability of tanks to hurt infantry.
Sure, a shell slap bang into the middle of a group of 5 grenadiers isn't gonna send them flying...

Also: Ragdoll physics which are incredibly impressive, but only when the dead.
i.e: 1 shotgun to the face - not even a flinch - another to the face and Mr Combine goes flying back...
Don't get me started on rockets...

CoD4,
the impossibility to have both a reflex sight and a silencer.

all C&C style RTS,
"We need you defeat the enemies of humanity, our entire war effort is geared towards their main base, and now we need you to lead the attack against their untold legions. Take these 4 riflemen, this tank and an engineer, good luck"

Crysis,
Ok, I know you've got an awesom battlesuit, but how far the designers thought ahead is obvious in the alien spaceship. For some reason, your armor comes equiped with zero-g maneuvering thrusters. why?! And where is all the fuel coming from?

any RPG,
You're about to safe a village/country/world faced with certain doom, that only you can avert. For some reason however, you still need to pay a decade worth of average pay for a new sword and some potions. After you're done saving the world, you still have to pay full charge just to spend the night...

Game: Halo: Combat Evolved
Difficulty: heroic
Weapon: pistol
Ammo: .5 inch (12.7 mm) Hi-explosive, semi-amorpeirce
Enemy: Elite
Procedure: shoot the elite until he dies, remeber to take cover.
Oddity: in some cases these guys can take > 45 rounds without dying (not because of his sheilds), i mean, that many shots could peirce 70 mm of steel (that steel being high quality co-fusion vanadium alloy, witch is HARD!)

Ok. HL2: HOW TEH F*** DOES THE SHOTGUN WORK??? i have been lying restless in my bed at night, restless, trying to figgure it out!
Advent Rising: if you auto target enemys and shoot at 'em, your aiming is shit, however if you switch to fist person mode, your assault rifle can headshot enemys at a distance of 132000 miles
Garrys Mod: the entire game.
LoZ OoT: you can as a 14-20 year old guy (depends on what you think Links age is) weild a freggin 3 m (10 ft.) long sword and swing it around?!?(i have estimated it to weigh about 25 kg (ca. 50 lbs.)), you would have to have biceps at the size of elefants to use that sword like that!!!
Heavenly Sword: Nariko is a girl estimate to weigh 60 kg (ca. 120 lbs.) and she weilds a freggin 3 inch thick blade, wich is 1.5 m long! that thing would weigh 20 kg! A THIRD OF HER WEIGHT!
Halo: How teh fuck can an average marine weild a pistol firering .50 cal hi-explosive rounds? And how does he compensate for the recoil of that 8 gauge shotgun??? tha things recoil could break your ribs or hip or whatever (depending how you fire it).
TF2: The engineer can repair stuff by hitting it??? Wonder what the medic gun fires? The demoman has invented the unbreakable glass bottle.
HL2 again: I would like to learn how to pick up ammo and healthkits from radioactive waste, through the steel bottom of an airboat, moving at 30 mph.
All futuristic games: WHERE ARE THE ROBOTS??? WHY DO WE SEND OUT GOOD MEN TO GET SLAUGTERED WHEN WE HAVE THE TECHNOLOGY TO MAKE ROBOT SOLDIERS?!?!?!?!?!

Let's see your question to Oot and HS, due to the fun of always using large swords they decide to give them the strength capable of weilding a sword that is possibly thicker than the density of a human skull and weighing as much as possibly thre armored trucks.

Did you miss the bit where Link picks up rocks almost the size of mountains at the end? I'm okay with him lifting the Goron Sword.

In my own experience with the HALO series.. I actually found playing at anything harder than easiest highly immersion breaking as it turned Master Chief from a highly trained, genetically superior super soldier of awesome to a fragile little infant trying to play with the grownups. Not quite Super enough for a Super Soldier.

How about Thief (or any stealth game really) -- random civilians walk around step step step, enemy guards trample about STOMP STOMP STOMP. You make half a step only vaguely louder than the absence of all sound and five hundred guards come running from a mile away "THIEF!! HOLD IT TAFFER!!". Okay, not really, but does Garrett have some kind of special boots that emit this highly recognizable I am a Thief sound with every footstep?

Of course we have the ever epic "Snake, Use the Keyboard!" (though the voice said second controller, disagreeing with the voice. Yes, I played the PC version). Immersion goes KABOOM!

I'm usually not too fazed, though every so often moments come along where all immersion is just torn to peices through blatant poking of game mechanics into your eye.

the game "Black" for the Xbox. (and PS2)

"Headshots with silenced weapons will not alert the enemies to your position!"

Too bad it's freaking IMPOSSABLE to get a good, clean, headshot.

Just where does Mr. Freeman keep all his guns? That HEV suit must have some freaking deep pockets if he can hold a rocket launcher, a SMG, a machine gun, 2 types of pistols, a crossbow and so on, not to mention the fact that whilst carrying enough guns to start a moderately armed militia force, he can still jump and run like he's butt naked.
That's kind of explained. If I recall correctly, in HL1 some scientist right at the beginning (might have been Kleiner, but I'm not sure about that) says the suit contains some servo units to support the wearer while moving, or something like that. I'm not certain what he said exactly, but that's quite close.

Brothers In Arms: Hells Highway, you have arms, legs, a whole body too! I find that pretty cool.

In Half-Life 2: You can kill someone by throwing pieces of wood at them repeatedly, but throwing a metal gun will have no effect (I'll forgive the instakill corpses on the basis that the Super Gravity Gun is more powerful).

Also, how the gravity gun is only supposed to affect inorganic matter - it isn't quite consistent. You can pick up wood (which is organic matter) and you can blast living antlions and headcrabs. But you can't manipulate corpses, nor can you affect anything humanoid.

Apart from that, the game tends to be internally consistent.
