ET for the 2600. Sorry, that one's kinda a conventional answer. Beyond that? I don't remember, I've played a lot of really bad games.

Sniper: Ghost Warrior.

The intro had such promise. A slightly different shooter, taking into account the wind and all that to make a single shot...

And then the very first mission turns into generic FPS #12753.

Pile. of. crap.

Worst? Ghost Busters on the NES
Most Disappointing? Halo Series

With all the hype it had I was expecting some super game tat was the best thing ever
but instead I got what felt to me to be a watered down version of Tribes 2, mixed with a retardedly simplistic and slow-paced version of Unreal Tournament...

My award sadly has to go to Duke Nukem Forever, my expectations were so high and all I got was a shitty Halo/CoD clone, and it's the only game I played where I was forcing myself to go on and PRAYING that it would have gotten better sooner or later....

...I stopped playing when I got to yet another turret section

I would say MW2, but at least you could survive more than 5 seconds in that game without getting demolished by a bulldog. Mickey Mouses Dangerous Chase takes it.

I'm stuck between saying Kane & Lynch 2 and Dragon Age: Origins (sorry, but I really can't stand that game, b*tch about em for it if you like).

Tetris.

Bores me like no other.
Doing the same thing over and over does not equate to good replay value to me.

Gothic 3 on PC

Wouldn't run well on my crysis able pc. laggy, woeful combat and an inability to aim at things. Then i left the town with no idea where to go and was immediatly savaged by a pack of about 20 wolves....
Yeah the combat was completely broken, I remember clearing out some mines in north full of orcs, by full of orcs I mean reducing the world population to roughly a half, only to get eaten by couple of stray dogs.
I believe that's the worst game I ever played and it's a shame, the first one was rly good.

Homefront... A game that starts great, then turns into a REALLY bad CoD clone after a deep and interesting intro level.

Evangelion: Kotetsu no Girlfriend (Girlfriend of Steel)

I know, I should have guessed it would be horrible. But I actually enjoy Japanese dating sims like True Love Story (I lived in Japan damned near a decade and speak/read pretty much fluently). I know that casts me in a bad light, but some of them are actually very well written. And the atmosphere of the games catches J-teen life quite well (frantic hellish study sessions, the desperate hunt for a disgustingly underpaid casual job, and the gaggingly difficult attempt for J-boys to meet the expectations of J-girls).

Kotetsu no Girlfriend was an exercise in name dropping. Any of the Evangelion games (generally from Broccoli, a developer associated with Gainax) that don't specifically deal with Angels and the whole impending 3rd Impact are generally about as fun as a flossing with piano wire. The Princess Maker-style 'Raising Rei' ("Rei"sing? lol) games were likewise just that - Princess Maker with different goddamn pictures.

Too Human. My then boyfriend made me play it. I wanted to beat his face in with a controller for it.
Is that why he's your ex-boyfriend?
No, but it may have contributed to it. (He was just a pretentious all round jerk.)

Farrenheit (I think it was called Indigo Prophecy for you Yanks). The first level was amazing, and promised a genuinely great game. There were a few hours after that where you start to feel like it's coasting on that good first impression, then... WTF? Out of nowhere you start getting these weird super powers, freaky hallucinogenic weirdness and the story went absolutely batshit crazy. Oh, and the 15 minute ling QTE sequence. I came out of the end of that with a very sour taste in my mouth.

captcha clearly fedice. Whatever...

Theres most likely been others but the one that comes straight to mind is -
Resident Evil: Gun survivor 2 on the ps2,

I always had a softspot for the 1st one, i thought this was gonna be awesome, only to discover the controls were awful, the storyline was a clone of Res. Evil Code Veronica although it was reduced to a skeleton of the original story, the game flucuated from super easy to rediculously hard in parts and the game got boring very fast.

Command & conquer 4. worst purchase i have made in 5 years
This would be the most recent game i bought and didnt like but atleast 75% of that is due to the online DRM and the other 25% being the storylines lack of attempt to actually provide any answers. They should have just called it C&C: Online and made it multiplayer only.

Big Rigs: Over the Road Racing. Its so bad people do not even speak of it anymore. It's the only game I know of that has received less the 1 out of 10 by a lot of game critics. Even though it was made in 2003 it looks like a DOS game. It's broken so it's impossible not to win since the opponent cannot move. There's a hand full of maps and trucks. It has several bugs that occur frequently which include:
player driving over 9000 km/h
driving in the sky
driving through the hills and houses

i am going to have to go with Call of Duty Black Ops.

that game was utterly dreadful...

no, i havent played too many bad games.
I believe it's the fact that you can just tell that Treyarch made the game. I'll bet that if you let Infinity Ward make all the Call of Duty games that Treyarch has made, they would play differently.

And I'm not saying I hate Treyarch's Call of Duty games, I myself love Black Ops but the feel of their games in both single and multiplayer just doesn't fell right.

Shepard's Crossing was a little piece of... nothing, for me. It was the worst thing I ever played solely due to the fact that I couldn't bear more than 30 minutes of it. Every other game I've ever played I've played for at least 3 hours before giving up.
