In-keeping with substituting numbers for words, they could do the inverse and title it 'Left 4 Dead Too'. If you're really worried about more, I'm sure they're just fall back on letter substitution. L3ft 4 D3ad. Le4t 4 De4d. Could even do some kind of shared letter kinda thing with the last one.
Regardless of the title, the only thing I'd really want to see has been touched upon previously- dynamic map generation. L4D already has an AI director, so why not create a Map Director and even better than just dynamically generating maps, dynamically generate the maps on the fly as you play, based on performance, just like the AI director does. Forgetting to check your angles? Spawn in some more vertical sections.
The only other idea that seemed any good would be some kind of overall meta-campaign. Anyone who knows of Urban Dead will understand the general concept, essentially a zombie apocalypse lightweight MMO, complete with scavenging during daylight, barricading and defending your safe house, or moving on to less scavenged or better defended locations.
Oh, and they really need one more "special" zombie- some excuse for the infected players to possess normal zombies while they're waiting on their spawn timer. Could be as simple as letting them just play a plain ol' normal zombie, or more insidious, such as gaining a following of nearby zombies and forming a potential horde, or becoming some kind of trap zombie that can be meticulously placed.
Actually, with that in mind, it'd be nice to see more in the way of interim 'species' of zombie. Mini-tanks, poppers, chokers, stalkers... zombies that can occur in similar numbers to the regular kind, either as packs or mixed in, with stats and abilities eschewed towards their specialist counter-parts. Poppers could spawn less additionals and only have a melee range pop attack, or maybe need to stack the bile effect until it's effective. Chokers have a shorter range, but no stun and can attack simultaneously- imagine a survivor getting strung up by a pack of chokers. Stalkers could have a wall-clinging ability perhaps. Zombies sticking to the walls and ceiling then dropping on you would work rather well.
