First specialist: I've used Tali(twice), Garrus, Legion and Jacob. All died.
First Fire Team: Samara. It shuts Miranda up so I consider it a safe bet.

Biotic specialist has never been a problem, I used miranda once and never again. Lost Thane which annoyed me no end.
Second fireteam: I've used Samara (twice), Grunt, Zaeed and Jacob. Jacob was the only one to survive.

Never lost an escort.
Only ever lost Miranda once and Tali once with the defence team.
The way I normally do it is as follows:

My Team: Legion and Garrus(Tali replaces Garrus post-vents)
The first specialist (vent dude): Tali
The first fire-team leader: Miranda
The second specialist (biotic): Jack
The second fire-team leader: Miranda or Garrus
The escort: Mordin (Mordin has a random chance to die right at the very end, no matter what, so always send him back)

Tech - Tali/Legion
leader - Garrus/Jacob/Miranda
Biotic - Samara/Jack
leader2 - Garrus/Jacob/Miranda
Escort - Mordin if you don't intend to use him in the last fight

make sure anyone you use (preferably even all of the people that you DONT use) are Loyal - only really feasible if you stick to either Renegade or Paragon 100% to get the required Charm or Intimidate. Don't use anyone else for any of the jobs. Either they'll die, or they'll get someone else killed.

Also make sure that you have all 3 upgrades, and work quickly through the final mission. Don't daudle.

If you go to the Collector Ship as soon as you get back to the Normandy, then it's possible to save every member of the crew (except maybe the one that dies in the cutscene showcasing how the refinement works), and all of your allies.

I have Garrus leading the second fire team and put Legion in the vents. With me I always take Miranda and Mordin. For the escort mission I put Jacob in charge and I keep Garrus for holding the line. Jack gets the role of biotic.

To make sure that no one dies you have to both have everything full upgraded but you also need to know the characters. Mordin says that he never cared for Kirahee's (sp?) 'Hold the Line' speeches and that he'd rather get the job done and go home, that's why he often dies if you leave him on the hold the line part. Samara has spent most of her life as a lone wolf and works best alone, so putting her as team leader is asking for trouble. The game tries to trick you by recommending someone with experience and most would go with Samara as she is the oldest by far, Garrus is a better choice because he has experience leading a team. Legion is a better tech than Tali so I put him in the tunnels. Jack a more powerful biotic than Miranda so she gets the biotic duty.

Basically you need to know the characters to get them all out safely, it's more than just earning their loyalty.

yeah, the game outright TRIES to trick you into killing people. It did that to me my first time through with Miranda as the Biotic and Grunt as the second team leader. He was like "Yeah, I got this!!" and Miranda assured me BEFORE I chose her that ANY biotic would do. Clearly not the one biotic that sucks >.>

Don't ask me my entire team died...
I didn't even think that was possible.
Recruit as few people as possible. Do as few loyalty missions as you can, but feel free to do: Jack's, Miranda's, Tali's, Legion's, Garrus's, Grunt's and Thane's. All of these people can be triggered to die manually over the course of the assault. Try to keep the rest of the crew unloyal. When given the choice, make Jack and Legion loyal instead of Miranda and Tali.

If you get Zaeed, get him killed in his Loyalty mission by taking the Paragon path and leaving him to die.

Use Miranda and Tali for the party leading upto the entrance to the collector base

Without any upgrades, this will cause Jack, Legion and Thane to die, regardless of Loyalty.

Use Garrus in the Vents, and use Mordin to lead the second team, Garrus dies, regardless of Loyalty

Use Grunt as the fireteam leader. He dies regardless of Loyalty

Don't send an escort with the crew, this causes all of them to die.

Use Miranda as the Biotic, bringing any extra Loyal people with you.. This will cause one of them to die, regardless of Loyalty. If you have 2 loyal people here.. well, I'm not sure what to tell you other then hope they die in the final scenes. If you have 1 remaining loyal person to kill off, try to save before the end of the barrier section, then load the game if they aren't the one randomly chosen to die.

Bring Miranda and Tali with you to fight the last boss, because of their disloyalty, they will die.

Hopefully, whoever you have holding the line remaining is either disloyal, or Mordin

If you have 1 or 0 party members remaining during the final scenes, shepard dies.

only if you try your best to get them dead which I did and then bioware announced that I can't import a dead shepard to ME3 so now I'm a little pissed...

just that one save. Just load your save from an earlier save, play the game through, make sure you keep 2 party members alive at the end, and Shepard survives. Viola, ME3.

like Weekend at Bernie's, but with Captain Anderson trying to convince the reapers Shepard is alive and still a threat by marionetting around his corpse.

You could probably sell that to Fox. Dying is permanent, you need at least two survivors or that Shepherd is untransferable.

It seems to be physically impossible to keep Jack alive. I have her in the team holding the line, she dies. I send her back with the crew, she dies. I take her with me to kill the Reaper, she dies. In all my attempts she remains the only team member that I have lost...

Sounds like she wasn't loyal. Did you do her mission? And if yes, did you choose Mirandas side during the argument later? If you want to keep both loyal there you have to use the para/rene option.

You have to fully upgrade the ship and gain everyone's loyalty and make non-retarded decisions with the assignments.
Also, when they're defending the door as you go to fight the final boss, it's best to take Jack and Mordin along with you because they are very likely to either die or endanger others.

I know. I've spent hours doing paragon actions in all the side missions and main story but I still can't get a high paragon score to access the paragon choice. It's the most annoying part of any game I've ever come across...

Shucks. I had Mordin die on me holding the line. Since he was my fav char that enraged me so i restarted the entire mission just to make sure he made it.

I did everything paragon and I didn't have any trouble in those moments but I would recommend leaving her loyalty mission towards the end, maybe the one you do before you go get the IFF? I usually leave that mission to the end myself so maybe thats why I never had any trouble getting both her and Miranda's loyalty.
