In that vein, I suggest we take the Starship Troopers route: if you want to conceive, you get a license to prove that you can at least probably qualified to raise a child. Base it on things like mental health history, criminal records (or lack thereof), or other gamebreaking factors, then move on to more flexible terms, like your existing number of children (meaning no more backwoods retards churning out a dozen kids when they don't even have the means to adequately support two), and state of employment/income.
