2. The radiation is already coming from James' sabotage as well as the heavily irradiated GECK (and I have no idea why I should have to explain that but there you are)

4. They went for a morality/destiny play and it didn't work out, they admitted as much and are fixing it. Personally I don't see the problem with that. I think we'd all love to be so lucky to be able to amend our less-than-spectacular endings.
Apparently nobody got to them on the idea of a second game in time initially, though reports are coming in that they may finally be working on it.
