Eh. Gore doesn't really faze me all that much.

I did shudder a bit in Hostel when that guy got his Achille's heel sliced.

Hrmm, for emotion I'd go for the pickaxe to the throat in "The Descent" (think I spelt that right...)
and for sheer ouch! factor, it'll have to be in "Death Race", during the fight scene in the garage when Statham slams that dude's head off the vice, man that was brutal... I almost felt it..
Oh and someone mentioned the whole 'barbed wire rape' thing from silent hill earlier, that's a good one too ^_^

Williams in "Enter the Dragon". This magnificent strutting peacock of a guy who says "When failure comes I won't even notice, I'll be too busy lookin' good" gets slowly beaten to death in a room full of beautiful laughing women, then boiled in a vat of opiate before being strung up naked. May not be the most gruesome but it's certainly one of the most memorable and disturbing deaths I've seen.

Mine would be the guy who had this alien proboscis rammed into his cranium and get the innards sucked right out from Starship Troopers. It was partly the cause of my insect phobia.

Akira, right at the end when that bloke turns into a giant gooey baby and you see his girlfriend trapped in the folds of flesh, then she just explodes into a shower of blood.

that girl who gets killed in her bed in the grudge 1, I know the grudge itself isnt a hugely scary movie but I've always thought of my bed as a kind of place of warmth and safety etc, so this one disturbed me a little.

While they got the grease thing right, in the graphic novel Rorschach handcuffs the guy to a boiler, pours gasoline everywere and burns the guy alive.
Actually, a dog gets a cleaver to the head, the deep fryer still happens, and the guy still gets handcuffed to the stove, but they cut that ALOT

Also, in Jurassic Park: Lost World, there was a guy in a jeep. One t-rex got his chest, the other got his legs, and the t-rexes ripped him in half.

Little children, wearing pumpkin heads, see a TV advert for The Evil Corporation (Silver Shamrock), and then bugs/snakes/spiders crawl out of their head, killing them, and then they eat the child's family.

That first one was the only part of the movie coming close to disturbing me. The second one, though, was rather hilarious (from narm) in the movie.

On topic, Asuka's death from End of Evangelion. If you've seen it or just browse the internet, you know what I'm talking about.

I really liked, and was haunted by the alternative (unflimed but storyboarded) ending to 'The Spirit' by Frank Miller.

The alternative ending had the Spirit rifling through the dead body of the Octopus, tearing it to shreads but still hearing the Octopus taunting him in his head and hearing his heart beating. The storyboarded sillouettes are pretty darn scary too.

I'm glad they took it out of the film but I've never been affected by voice-overs and black and white sketches so deeply (see DVD special features). It would've been a powerful scene in the right movie (Like the Tell-Tale Heart for example).

Pretty much all of Rambo IV.
The way they merrily tear apart hundreds of people, it's no worse than the violence in Saving Private Ryan. It's the way it completely dehumanises all the bad guy and reduces them to little more than meat chunks. I think the film would have been better called 'racist yellow gook wipeout'.

The comic is worse than that.
<spoiler=in the film, instead of a cleaver> Rorschach doesn't kill him, he handcuffs him to the stove and sets fire to the house

probably burning alive, just knowing your alive for almost the whole thing is enough to make people poop their pants.

EDIT - oh i got another, having your face sanded off either with a sander or brute force like in the Hellsing manga when Ceres kills Zorin Blitz by grabbing her by the head and dragging her accross a wall face first.

Most of the time it's not so bad as you kinda see it coming, but in other films, meant for families I always find it worse. Like some of the deaths in Indy 4, ants, and burnings, and so on.

Many Takashi Miike - like Ôdishon with the cheese wire cutting the feet off or one of the DOA's where they pump the girl full of drugs and sodomize her so much she's left in a swimming pool of her own feces and then stand on her head to hold it under.

And just reading about Gurotesuku has the hair on my skin on end..
