Its about addiction and how badly it effects peoples lives. It is by far one of the best films ever made. Plus I wouldn't call it disturbing, its a masterpiece, chokes me up at the end...
Also if you read the book the fridge talks to her about eating all the food, see its not actually attacking her, its just what the drugs have done to her mind.

Irreversible - Its a poor film in truth, but it by far has two of the most intense disturbing scenes of all time.
Visitor Q - Starts off with some incest and to be honest, just gets more messed up.
The Shining and The Thing - Classics mate!
Like mentioned Cannibal Holocaust and Audition are pretty disturbing.

If you want something more disturbing you should check the directors other films, they are a lot more gritty and realistic and I'd say better.
Which reminds me Three Extremes is up there with the most disturbing things I've ever seen, its three 20 or so minute films from three Asian Directors, Takashi Miike who directed Audition and Ichi the Killer (which is a black comedy I think its over rated with the whole "disturbed" thing), Park Chan-Wook (the guy who did I'm a Cyborg and Oldboy) and a shorter version of Fruit Chan's Dumplings, well worth the watch!
