The scene in the orphanage in The Darkness where Jackie's girlfriend is shot, the Darknesses words there are creepy as hell..(he's technically an enemy.) And then the entire section in The Darknesses own personal hell afterwards, with the fucking scary zombie Nazis that pop up in the screen if you stray off the path.

Dividers from Dead Space. Just don't know what it is...well ok I lie, there sound effects are a bit freaky and their appearance coupled with the fact they can split off into individual creatures makes me shiver a little...but theres something else that I can't put my finger on that makes them creepy, maybe the tall, thin appearance
Vampires: Bloodlines had this really creapy thing that was four naked old women sewn together at the hips, three of them acting as legs of some kind, and the fouth acting as a clawed monstosity that screams at you and chops away half your health... the graphics of the game didnt make it that bad but what does is imagining it done in current graphics.
oh, and most the stuff in the upcoming Dante's Inferno game will blow any and all of the things listed here out of the water i am sure...

Most people say pyramid head, but seeing as I've never played a silent hill game...... I'm gonna have to go with Alma from F.E.A.R.
EDIT: Strangely, I've only seen one other person who said Alma.... I feel weird, but seriously the fast zombies from HL2 were pretty disturbing.
