I would go for the complete final level of Doom II, the enemy being the horned wall and all the previous final bosses that it spawned.

I don't know if anyone's said this but the hybrids from SS2 were creepy. running up to you saying 'kill me...' and 'I'm sorry!' Hated hearing them talk, it was so freaky, especially early on when you don't know why.

And if anyone hasn't played AvP and says that there is anything that is more likely to have you swapping your underwear at regular intervals I put it to them to play it and then give their verdict.

see now this is when game designers wanted you to struggle through the last boss and didn't care if it meant that you didn't complete the game.... I've tried it on nightmare (it didn't work... I died... a lot

i have to say theres loads of stuff that disturbs me in Fallout 3

the worst being the crying IEDs in the baby carriages, that is downright horrible

also the sound of Deathclaws breathing and ghouls running, or the point where you find a bunch of friendlies in the middle of the night who will suddenly slaughter each other over a fridge of water..

the base humanity that appears in that game is enough to be very, VERY unsettling.

If you could wake up the big one and then stay hidden and wait a long time, it would inevitably throw a spawn cube at a spawn target which still had a monster on it, and they didn't telefrag. They jammed. Wait long enough and all the spawn-spots become jammed masses of creature, and you can just stay out of claw range and concentrate on the developer's head on a stick down a hole behind that hole in the wall.

Not sure if its been said, but Dr. Steinman from Bioshock was sketched out. Just before you boss fight him where he has all those mutilated bodies with bags over their heads and that creepy audio diary you get after beating him.... Too disturbing
