Probably the zombies in Thief: the Dark Project. Especially since they're almost impossible to kill. The stalkers in HL2 were pretty disturbing as well, but something about those zombies just freaks the fuck out of me, even now.

How could nobody forget this fucking thing from Silent Hill 1!!!!!

It wasn't really an enemy. But it was a monster that u couldnt see, but it would 1 hit kill you. In one particular room, if u didnt put the key in right when You went to grab the item u needed, it just reached out with its creepy black aura, and pulled you under the things and did God knows what to you. ugh, if anyone can find a picture or a video of this thing i would love to see it

or in tf2 the scout (with the Sandman (STUN...) or the Force -a- Nature (kickback) or the bloody infulnarable drink BOINK) its frustrating
Dont forget that the gravity gun tends to be a waste of time on them.
When I was little and I started playing Morrowind the picture of a dreugh freaked me out.

Probably either splicers or little sisters. Something about the splicers singing hymms and the pitiful cries of the little sisters really disturbs me.

The splicers in Bioshock (because everybody loves deranged 60's British singing mask wearing rejects with claws coming at you).

Those... things... in System Shock 2 (the graphics make them look even more terrifying).

Finally, for a little bit of light humour, the head crab zombies from Half Life 2. Why? Because the sounds they make, I find, are absolutely hilarious. I still laugh at it to this day. At first, I was pretty scared, of course knowing how much indescribable pain they must be in, but then they became funny. I can't take their shrill screaming seriously any more XD The poison head crabs are no fun at all though 8(

Necromorphs from Dead Space. First time I shot it in the head, it went down. I was like "Oh that was sweet" and started walking towards it to get to the door, then RIGHT IN FRONT OF ME IT JUMPS UP! And it's all like "OH HAI THAR" Then I proceeded to shoot randomly until it finally died XD THEN the what's his name black guy told me "body shots don't work, you need to dismember them"

Oh fantastic, you tell me, now, after I nearly get my head sliced off @.@

Pyramid Head, the nurses from Silent Hill 2 (God I hate the hospital >-<)

Issac always seems to die in the most horrendous ways. He's really unlucky.

The Poison Zombies from Half Life 2. Their hulking form and piggyback Headcrabs make me drop everything and attempt to take it down while trying to keep myself from screaming in fear.

The redheads in Ocarina of Time creeped me out the first time I stepped out of the Temple of Time.

The very first time you meet the Flood in Halo also creeped me out. Didn't see them coming and they were pretty much the only enemies that made me feel weak as a player. Every fight with them would result in me backpedaling and running just to survive. That was kinda lost in the sequels so meh.

More recently the necros in Dead Space. They kinda lost their shock value as the game went on but they were definitely creepy as hell.

First time I saw a centaur in Fallout 3 I freaked out but after that they weren't as shocking anymore.

However the Iron Maidens scared the pants off of me, each and every time, to the point I quit my 3rd polythrough when I got to that area.

Not from a game, but the Silent Hill movie(which sucked),the scene with the lava babies, the sound that they made; to me, it was just horrific.

Idk, that skin ripping scene was pretty epic... even though Pyramid Head really had no reason to be in that movie...
