Carlos in Saints Row 2, when the boss pulled out his pistol I just started yelling shit again and again, Carlos was an awesome dude, what a nasty way to end it.
Arkham Asylum, first scarecrow encounter in the morgue made me drop the controller and lie on my bed shaking for about 5 minutes, certainly was not expecting that out of a Batman game! But I probably shouldn't have been playing it at night.
Red Dead Redemption, yeah a lot of people say it, I guess it's testimony to how much of an impact it is, after all I felt a real connection to John, to see that happen to him tugged at my heartstrings.
Far Cry 2, Listening to the Jackal tapes made my jaw drop, as it hit home then that I had become more than just a nameless mercenary, I had become a mass murderer like the Jackal himself.

Heavy Rain: Madi awakes tied to a table, with a creepy old man saying he's going to operate on her whilst she's still concious. And her being forced to give a man a lapdance at gunpoint.
Final Fantasy 13: Vanille breaking down crying and begging Sazh to shoot her.
Rule of Rose: Amanda holding a live rat against Jennifer's face, and in the next chapter, Jennifer being forced into holding the same rat, that is now dead and rotting against Amanda's face.
Bioshock: Ryan's death scene.
Clock Tower 3: A man sprays acid into a man and his mother's eyes, dumps them into a metal barrel, then pours acid on them.

A lot of people don't like it... Quake 4's "Medical Facility" was pretty gruesome. Pails in comparison to some of the stuff in this thread, and of course it was completely spoiled by almost every review of the game ever, and even the back of the box... so... probably not too much of a surprise it didn't do too well...

No game moment has ever made me squirm more than Dead Space 2's "Eyeball scene". Only part of that game to freak me out.

The torture chambers in Amnesia were pretty damn gruesome, even though it was only described in text, the machines just looked horrific.

When your partner gets downed in the basement in Perseus Mandate F.E.A.R. That has to be the most realistic blood curdling scream I've ever heard.
