I'd have to go with the plot-twist/plot-reveal of The Machinist. The movie sets it up from the first moment, yet no one I have talked to saw it coming. Then again, The Machinist is a superb movie.
what the whole Miller / Killer thing... that was a little easy to pick up :p and that ... hehe watch the movie it's fantastic, possibly Christian Bales best movie, which is saying a lot.

Kevin Spacey is...

...actually, if I tell you he'll kill me, you too and our families, in fact I've said too much already.

I watched oldboy a couple of weeks ago. Man...that film wouldn't have gotten made here. But I recommend it if you like having your mind blown.
I second his post in all possible ways.Oldboy is just simply amazing. And the twist he's talking about just comes out of nowhere and hits in the face, but in the best way possible.

Fight Club anyday. If you rewatch it and try and spot all the little things they did to make the twist make sense you will really see the genius of the director. It must have been a complete nightmare to film it.

Use of Weapons.

It's a little-known SF novel by Iain Banks, and if I had to make a list of my favourite novels of all time, it would probably be on it. I literally had to put the book down and take a few minutes to recover after the final twist. It completely blew me away.
I completely agree, most original surprising book I've ever read, I still get shocked by the ending when I re-read it. It's definitely in my all time top ten books. Try reading other ones by him like 'Against a dark background' or 'the player of games' or 'state of the art' all have pretty awesome twists (though not quite as good as use of weapons)

Dexter, series 4 :(

Dexter spends the episode sending his wife and kids away so that they'll be safe from the Trinity killer, who has worked out Dexter's real identity. His kids go to Disneyland with their grandparents, and he convinces his wife to go on holiday to the Florida Keys with him and their baby son, with Dexter meeting up with them after he's "taken care" of some business. He then proceeds to track down Trinity, eventually finding him, and Trinity's last word are "It's already over", with a sad look in his eye...

Exhausted, Dexter walks into his home and sits at the kitchen table after disposing of Trinity's body. Smiling to himself, he takes his phone out of his pocket and turns it on to find one new voicemail message. It's Rita, telling him she left her passport at home and that she is coming back home to get it. Worried, Dexter rings her phone only to hear it ringing on the kitchen counter, and his son then starts to cry from down the hallway. He runs into the bathroom, to find his wife in bathwater tinted red by her own blood, and his son sitting in the overflow on the floor

Some very good picks so far that i agree with. So I'll just spend the time and say one i haven't seen yet which was The Prestige. One of the greatest movie endings ever.

Umm I was stunned from the ending of the first Saw movie and the remade version of Oceans 11.

That bit in Shrek 2 where it turns out Darth Vader was Keyser Soze all along, and that he was dead THE WHOLE TIME

Metal Gear Solid 1. The others were also good, but this one really blew me away. There are many of them in the MGS series, but the first one did it for me completely.

This this this. If anyone ever tries to tell me they figured out that twist before the end of the movie, I immediately dismiss them as a liar and kick them in the jimmies. Easily the most amazing movie twist ever.

Soylent Green is People!

also, The Game starring Michael Douglas. That movie is a headfuck from start to finish.

The funniest spoiler due to bad acting HAS to be Soylent Green

"It's made out of people.YOU GOTTA TELL THEM IT'S MADE OUT OF PEEEEEOPLLLLLLLLLLLE!"
You can watch it right here: http://www.youtube.com/watch?v=8Sp-VFBbjpE.

Funny thing is that's the end of the movie. When the guys arm sticks in the air like in the box art of Left 4 Dead

I just saved you from watching the whole movie

Every episode of Lost, Heroes and Spooks. Oh, and the several unexpected deaths in American McGee's Alice.
