I was messing around in Portal once. Shot one on the ground in front of me, one directly above me. I then proceeded to drop a live turret through the one on the ground, depositing it (still shooting frantically) onto my head. The initial hit damaged me, and the barrage of bullets finished me off.

I can't say it was entirely unexpected... But it WAS pretty pointless.

i walked off a like 3 foot fall in prince of persia:the two thrones and i died

at about that time i quit PoP anything

Planting a mine, and then dying, and then setting off the mine.

And spawning into a vehicle parked over a spawn point and jittering off the bottom of the map.

CSS: I walk into room, room explodes with gunfire, I'm the only survivor, there one badguy left, The hostage is wih me. I am inches from the hostage point annnnd. BOOOM HEADSHOT!!!!!

I still love WICKED SICK!!!!!!

1)The CPU or anyone on Xbox Live, constantly using the same move on Soul Calibur 4 or any other fighting game, which picks you up of the ground, hurts you and drops you down again repeatedly.
2)Falling through graphics on games, like WoW, or getting stuck in the graphics, for example in gears of War.
3)In any game which has a driving sequence, and you can have an AI controlling the vehicle, while your controlling the turret and drives stright into fire by the enemy.
4) On Persona 3, the enemy uses either Mudo or Hama(or the higher level versions), gets lucky, the main characters dies(You get a gameover if the main character dies... :|) and game over and then having to start back 10floors below..... (Shortly after that i found out the best way to avoid losing floor progress is to run to the next save point floor, and then go back down and go up normally to level up... :x)
5) On FPS' when your hidden behind something, really far away and the enemies seem to have super vision and extra long range fire and managed to kill you from a mile or 2 away.. :Z
5) On a platformer, the camera angle suddenly switches and you end up jumping off the cliff instead of up the step or whatever. :X

I could think of more, but i'll refrain. :p

When I first got to Vermire in Mass Effect I saw this opening between the rocks on the edge of the map, now I totally knew that if I continued to march on I would fall to my death, but curiosity took the best of me so I carried on and eventually fell into oblivion. The problem with this was not the death itself but the fact that for some reason the game didn't recognize my saves made me have to start the level from scratch. Ridiculous.

full hp, body armor, everything some police hummer runs me over and smashes me into a wall

GTA San Andreas:
I was exploring in Las Venturas and i went into a shalow pool to pick up a horseshoe. When i jumped out Carl slipped, fell and broke his neck. Or at least that's what i think happened. Full health gone instantly. This only happened once.
Battlefield 2:
The typical situation when i try to get into a tank at the begining of the round but a team member beats me to it and decides to go in reverse and run me over.(single not multyplayer)
Gothic 3:
The inferior wild boar knocks me down and hits me continuously and i can't do a thing about it.

Last boss in Tekken 5 story mode. I'll just put the three most pointless deaths here.(read: how 75% of my fights against the guy end)

1 - I attack, he stuns me a few frames before I hit, throws me away and charges his 70% damage laser type attack.
2 - He immediately stuns me, and I die before I can do anything because he launches his 70% damage laser twice.
3 - The most pointless and "smash my controller" death would be getting him to 50% health with me having 100% left and then getting hit a few times by his attack that heals himself(stun > heal > stun > heal > stun...) and he gets a "Perfect" for doing that.

*note*
The above is against the final boss on EASY. Yes, I suck at playing fighting games, but I shouldn't get slaughtered on easy. I even had to enable 1 round wins because I usually managed to win 1 round but then get slaughtered in the following two rounds. Also, the 70% is an estimate, could do less damage but it doesn't matter because it's almost impossible to dodge the attack.

[Edit] Yes, I know, why buy a game I hate? Well, I didn't buy it, I got it when I bought my Ps2.

While playing Dragon Quest for ps2 (i dont play it anymore, but i played it back then) i was walking around, then i find an extremely powerful creature wich came along with two shrooms. angelo and jessica were dead, so it was only me and Yangus. i made an extremely strategic plan to kill the beast, causing the death of yangus during battle. when i won, the last remaining shroom killed me with a week spell. bullcrap.

Shadow of the Colossus on hard, 16th one

After taking about half an hour trying and failing to get up to the bastard, I get to his head and get into a PERFECT rhythm of stabbing and waiting.

Then when I get him into about 1 more stab of health left (seewhatididthere) my grip meter fails and I go plummeting to my death.
Then when I get back up to him again, I can never get that rhythm right again T_T.
