AoC walking along on a hill side trail and getting kicked by a horse and falling off a cliff to my death
I died trying to drive a motorcycle down the biggest mountain in GTA:SA. about 1/4 down the hill, I flew off the bike and fell to the ground. I was amazed when i realized i survived, and then about half a second later the bike fell on my head and killed me.
I even got a screenshot of it happening.

In Demon's Souls there's a giant dragon boss. It is huge and can shoot flames out of it's mouth. Well, it killed me... by punching. Literally, a fist the size of a bus lol. I just think it's ridiculous to die by having a fire breathing dragon punch you.
Same thing happened to me in Just Cause 2. I had full health when I jumped onto a foot-high ledge. I died.Woodsey said:

I had something like that happen in campaign on the Parrish. It was at the bit where you go down from the broken freeway to the cemetery. I tried to jump on a hunter that was below me but it jumped up and I'm pretty sure I hit the games invisible cieling. Throwing stuff at people in GTA4 does damage. If you have the patience, you can kill someone by throwing a box of chips or a bottle of sauce at them repeatedly. Ideally, you need someone who can't run off, like people hemmed in by restaurant counters: I suggest the Burger Shot in the bowling alley in Algonquin. It only takes about fifteen throws.

I find it funny when ya do a u-turn when flying due to invisible boundaries and crash into a building or rock XD
Or when ya get killed by accidentally throwing a grenade at a wall or in the air, or stepping on you're own land mine ya forgot about.
Once I was playing Red dead redemption and I was riding on my horse escaping a $1800 or higher bounty, when suddenly my horse jumped off a bridges into the river separating Mexico from the U.S. because some signs (probably saying broken bridge idiot) or something blocked my view of the abyssal fall ahead of me XD DEAD!
