In Crysis, many a Korean soldier met death via the means of chicken/frog-meets-face. I can only imagine the funerals they must have had.

I had just walked down a ladder onto a platform that was probably about 4ftX4ft. I stopped for about five seconds to admire the burning corpses hanging from the ceiling and the apparently bottomless pit that surrounded my little piece of floor. I turned to walk back up and came face to face with a Daedroth, but I wasn't worried, I was fairly high lvl and had a spell that could knock it out in one shot.
Before I could switch to it, the over-sized alligator backhanded me clean off the platform, and I got a nice 3rd person view of me falling into the darkness.

It wasn't me but I was playing gta 4 and I was jacking a car while trying to escape the police. I jack the car but get back out. While I was jacking the car the police did the same thing to some random npc. Then he also got out and proceded to shot my direction killing the npc he draged out of the car.
San Andreas, doing a drive-by on another car at close range.
And then a stray bullet hits the other car's gas tank. It blows up instantly, and now MY car is on fire!
"Oh shit!" I say, mashing the exit vehicle button. But my car explodes half a second later. Wasted.

In Socom, we like to plant claymore around the hostages in extraction, and I like to make a trail of them, so they shoot the one claymore thinking they're smart, but really it'll blow them all up in a chain and kill the hostages they're trying to rescue. Well me and my buddy got creative and made a chain of 8 claymores, and as I was laying the last one, this guy rushing shot the first one and killed me but not the hostages =/
In Red Faction: Guerilla i was killed by a strong contender for "Most Badass Mook" once. A lowly EDF soldier (just like the thousands more i had killed and would kill) single handedly took out my armored assault vehicle by climbing atop it and planting a Singularity right on top of me.
I was furious i had died but couldnt stop laughing at the abusudity of it all.

there is no 'definitive' ridiculous kill, but some of my deaths include
having a sentry bot and a mr gutsy ambush me when i'm frickin level 5
run into a death claw after I FIRST LEFT THE VAULT
getting killed by a raider I didn't see for about 2 minutes
having a raider run into me at evergreen mills and whack me so hard I fall off a cliff
RADIATION grrrr... enough said
anyway

I can't remember which game it was, but I do remember that happening. Probably Fallout or Crysis. Those games are whack.

Falling off of hip-height objects, drowning in water that you could walk across, ANYTHING in GTA4 (tripping downstairs, getting shot in the head while at the wheel by a guy in a car travelling in the opposite direction from you at 60mph, getting clipped by traffic and dying of apparent cardiac arrest, etc).
EDIT: Either that, or in San Andreas when I labouriously drove a nice shiny quadbike I'd found in the middle of the countryside all the way back to the safehouse next to Mad Dogg's mansion, being incredibly careful not to hit anything, when I got just over the crest of the hill by the "Vinewood" sign and got hit head-on by a weirdly low-flying bi-plane and died instantly, exploding all over the hillside on impact. I didn't know whether to laugh or cry. In the end I think I did both.
