This game haunted my childhood.

Kudos to anyone who recognizes it.

Hard question... I would say first time i saw a headcrab in Half-life 1... Long time ago... That kinda freaked me out.
I'd already seen my friend playing the first bit of the game (which is why I bought it), so I expected them. The fast moving ones freaked me out the most, plus the ones where they were on the people's heads or something... *shudders*

Nothing.

My problem is that I've been watching horror movies since I was 6 or 7 years old.

Damn, ninja...

OT: I agree with the above post, although I think unnerving is a better word than scary. I find enemies you can't kill rather unsettling.

On the later levels of "Rebel Moon Rising" (Obscure game, I know, but it plays like Doom), you get cyborg enemies which have a nasty habit hiding in shadows, or waiting round corners to surprise you. Generally speaking, they can kill you before you can even say "F***", let alone before you can fire.
Whoever took this screen, I'll bet they died shortly after.

I was about five or six, when I first played it o_O

Have to say though, Deathclaws look nasty.

The Dead Hands from LoZ OoT.

The first time I encountered one, I imminently paused and ran out of my room scared. (this is when I was young)

And that giant buff mutant thing from FarCry.
Sounds like me, but I stopped playing when I saw those zombies when you turn into adult Link.

The bear in Condemned scared me only because I never expected a raging bear to be in a game like it. It had my heart racing as it chased me.

oh god, yes. the first time i saw one, i unloaded a full clip of TMP ammo into the bastard, and... nothing. he only died when the door slammed on him, kinda an exploit, but it's what i take when i'm not equipped properly.

another thing, when i was watching my friend play ocarina of time, he went into this crypt, and there were these weird zombie things that latched on to you if they saw you, holding you in place and sucking your life away... those things were creepy.

Yeah Alien 3 on the SNES.
I had the same (or similar) game on the sega megadrive. I hated those damn aliens.

MissingNo scared me, first time I saw it. (I was using the exploit but wasn't aware of what was coming.) I thought I'd broken the game or something.

The Half-Life Gargantua was pretty gnarly on first encounter, just because I rounded a corner and it was right there.

In Bioshock, that crazed doctor that stands RIGHT BEHIND YOU when you go to pick up the tonic in that steam-filled room. Nearly shit myself when that happened.

I'd probably go with the Regenerators in RE 4, by far the only enemy in that game that gets the fear going. Gonna throw in a mention to Nemesis from RE 3 though, at the age of about 11 that guy/thing was terrifying.
