unfortunately i haven't played many horror games the most scary creature i faced was from turok 2 for the 64 basically any creature in that game but the first lvl scarred the bejesus out of me with the raptors running out of the dark horizon to carve ur face up

Maybe I'm just a coward, but Demons from Doom always make me shit my pants. Especially the one that comes charging at you in a hallway in Hell Keep.

I never though that Giygas was scary, though. Territorial Oaks however... Maybe it's just me, but a tree that explodes and kills all your characters is freaky as hell.

I don't know, what defines scary is radically different between people, so the Rabites from Secret of Mana might be more terrifying than the end boss on Doom 3.

The most disturbing recent stuff that chills me some (if they attacked in real life, but thanks to physics and biologu, can't) Dead Space and some stuff by Alex Pardee.

I always wondered what it was. But that portal scene in Half-Life 2 has almost scared me. Or, ather, the Ichthyosaur that swims into your face. *shudders*

The beginning of Ravenholm from HL 2 was also kinda scary.. but that's not a monster..
Oh and the ghosts in one of the vaults of Fallout 3 if you can count them. (Ghosts xD)

I played OoT when I was little. As in four or five. I couldn't beat the puzzle in the Great Deku Tree XD. Regenerator always scares the piss out of me. The first time I met him, I scored the infrared scope and said "sweet!" out loud. I heard raspy breathing, but saw nothing. I decided to make a mad dash for the door, not realizing that that big beast was RIGHT FUCKING THERE. He grabbed me, made me wet myself a little, and owned me in a gruesome, terrible way. I then played Shadow of the Collossus.

yeah when you first step through to Ravenholm it has an eerie atmosphere. I think the atmosphere in games can be the most scariest. I mean Painkiller's town map doesn't have particular monsters that are scary but the atmosphere and sound effects are so creepy. F.E.A.R has some creepy moments in its level design for that matter

Hunters (L4D) are the only video game monster to make me jump. It was more out of surprise than genuine fear, but that's as good as it gets with me.

i did do a search and your thread "most disturbing enemy" didn't show(i think i searched for scary game moment or something) anywho, its not really a copy as this isn't just about scary monsters/creatures, but scary parts of a game. like scary bits of level design or set-peices and things like that :)
