Personally, the most surprising for me was in Call of Duty 4.
When the nuke went off I was seriously was startled by the explosion, and I caught my self swearing in panic. I did not see that coming.

As for the saddest death, I think it is a split between
when you think that Atlas' family was just killed.
That made me cry a bit. That just made me even more angry when you found out the truth. That was great storytelling.

The other was
when Eli died in Episode 2.
Jesus. I loved that guy! When you develop such a strong relationship with a character like that, you can't help but feel sad.

Anyways, Agro and Wander's "death" was pretty surprising.

For me it was Aeris, but I played FF7 when it was newish, and as such hadn't heard a thing about it. surprising cause she was a main character, sad.. mostly because of the music.

Some of Lost Odyssey's "Thousand Years of Dreams" stories were sad as well, I think it was the first one or one of the early ones had me sobbing like a little kid.

The Darkness

Jackies girlfriend gets blown away right in front of you and the Darkness keeps you from doing anything to stop it. The ending, although it was not really a death, was really sad as well.

I must admit though I didn't like The Boss from MGS3 most of the game, her death was very impacting.

Now, the saddest deaths...
Alice and Yuri from Shadow Hearts. Alice because you go through all that to save her and you see the story between her and Yuri totally bloom and their relationship and stuff...only to lose her in the end.
Yuri's because, well, he was awesome and his death is kind of out of nowhere...and also just plain sad.

Caim and Angelus in Drakengard 2. C'mon, they're the main characters of the first game and now you just caused them to die! That's so not fair. -_-; Definitely made me cry.

Reks in Final Fantasy XII. Warms you up to the character, only to kill him 20 minutes later.

Zack from Final Fantasy VII Crisis Core. Just because you know it happens doesn't mean it isn't freaking sad. :(

Steve from Resident Evil Code Veronica. Yes, I found his death to be very sad, thank you very much.

Emma from Metal Gear Solid 2. She's not even in the game long enough for you to become really attached but I found her death to be much sadder than I was expecting (already knew she was going to die).

Huge warcraft three fan so im gonna have to say Grom Hellscream just because he was awesome it was unexpected but most of all coz i felt cheated that he defeated Mannoroth and still died.

Silent Hill 1 / Fallout 3:

Lisa Garland in the original Silent Hill. It crushed me as a little girl, and her death scene still gets to me. More recently, Dad in Fallout 3. I cried. A lot. Some jackass on here ruined it for me, I knew when and how it would happen, but I still just wasn't ready. :'(

Definitely Shinjro's death, he was a badass with a heart of gold. Luckily if you level him up on a new game plus he learns Null Pierce. To a lesser extent Chidori's death was pretty sad to. Also the MC's death really surprised me because it takes balls to off your MC and he held on for a whole month just to say good bye.

General Leo, The guy was a epitomy of the honor and awesomeness. His death made me finally start cursing Kefka's names along with his murders at Doma

Ragou's death really surprised me, I figured he would eventually get hauled off to jail but no Yuri goes and kills the uppity bastard.

When Arthas became a Death Knight and eventually the Lich King himself. His body never actually died, but nothing of the original Arthas remained at the end of Warcraft 3: The Frozen Throne. In a way, he did die. Not in the sense of spiritual or physical death tho. I know the moment Arthas became a Death Knight was expected; it was no shock when he betrayed his kingdom and became a Death Knight. But to watch his transformation, to see how he went from a light-bearing paladin to a murderous, soul hungry, Death Knight was very intriguing.
