Wallmasters and Floormasters from Ocarina of Time.
Pretty much this.
I remember finding them on my first play through of Ocarina of Time when I was 7. They scared the freaking hell out of me.

What, this far and still no mention of Plaster Splicers? I can't be the only one freaked out by them.

The Ritual Monster from Silent Hill 2.

An invisible monster in the game that can't harm you.

You're walking down a dark corridor in a prison; cells on either sides of you. Suddenly, your radio crackles to life, a sure sign that a monster is near. You shine your flashlight in to the darkness, but you can't see anything. As the radio grows louder, you can discern footsteps coming towards you. But you still can't see anything in the darkness.

The radio grows louder and louder, and the footsteps keep coming towards you in a steady pace.

They pass. You still can't see anything. The radio starts to quiet down.

The footsteps stop. A low, male whisper utters a single word; 'Ritual'.

Then the footsteps continue in the opposite direction.

To this day; the noise of that one monster terrifies me so much that I've had to tell my friends to not play that noise for me. Even as a joke. It can seriously send me right in to an anxiety attack.

Facehuggers, they're tiny, quick, can scale walls and one hit kill you by humping your face unless you have the reflexes of a ninja.
The sounds of their scuttling and forever burned deep into my pysche.

The Cyberdemon in the Doom series.

This guy can destroy you in 2 shots and takes a lot of punishment to kill.

I hated those poison headcrabs in hl2. they freaked me the fuck out. not the health prospect but just the size and the slow speed of the bloody things, and then there's the fact they're FUCKING THROWN AT YOU to boot. Ugh

Every enemy from Painkiller:Battle Out of Hell's Orphanage level. If I recall, I've played that level two or three times in the entirety of the three years I've owned Painkiller.

The Regenerators from RE4. Powerful attacks, nearly invincible and utterly terrifying to look at.

this. regenerators breathing scared me so badly. not seeing them, but knowing they are around. and then dramatic music starts, and breathing gets louder, and you still can't see them. so you break into a run, try to get out of the room and BAM!

regenerator in the doorway...

How have Deathclaws not been mentioned yet?
Everyone's, and I mean EVERYONE'S, first thought when these pop up on screen is "FFFFFFFFFFFFFUUUUUUUUUUUUUUUUUUUUUUUUCCCCCCCCCCCCCCCCKKKKKKKKKKK!!!!!!!!!!!"

Seriously, fuck those guys.
Agreed, and are you taking him on bare handed? if so, tip of the hat to you sir!
Oh that's not me, I'd be too scared to. My policy is literally "Nuke on sight.", and that's exactly what I do.

Deathclaws in Fallout 3 were pretty intimidating. When I first saw them I didn;t know what to do, they're fast and strong...until I got faster and stronger!

Gargantua.
Half-Life.
Sure, not that scary now, but when I first played Half-Life, it was terrifying.
That scene when you have to run past it and down a corridor with the thing chasing you...
It's the enemy that came closest to making me shit my pants.
