The poison blade in AC2, instead you could have stabbed the guy with the normal blade and got it over with. Fast.

The Wabbajack Daedric artifact in Oblivion, it didn't produce any lulz, it actually almost killed me because it turned one scamp into an ogre and another into a Daedroth, holy shit.
