Not really a death, but I'm still horrified to this day because of what happened to Casca in the Berserk manga. She was once a battle commander for the a legendary mercenary group. Then she was broken. Never mind that said legendary mercenary group was obliterated by cosmic horrors and eldritch abominations during the same day. What happened to Casca is one of the saddest events I saw in any medium.

EDIT:
Grave of the Fireflies. Just that. It had me weeping by the last few minutes of the movie.

Movies:The end of "The Crow"

Books: Taretha Foxton from Lord of the Clans.. So. Fucking. Sad.

Anime: I usually hate anime, but I tend to love all things cyberpunk. So when Motoko "dies" and Bato yells her name was really sad. Oh, and don't forget the death of those think tanks. Oh my god.

Games: For the single longest twenty seconds ever, when I thought Commander Shepard died and I was on the edge of my seat.

Apologies for the necro, but I know how much people hate repeat topics, and this is something I really felt I needed to put on one of these topics.

I just finished watching the newest episode of Full Metal Alchemist: Brotherhood on Youtube, and the death of
Lt. Colonel Maes Hughes
was a huge blow to the tear ducts for me. I mean, they go out of their way to show how much he cares for his family and how good-natured of a guy he is overall...then they have him get shot to death in a phone booth. That...was just messed up.
