After playing Assassin's Creed, I started to see my ancestors, but they didn't stay longer than 30 seconds so it was fine.
Seriously, no, but I had many cases where I thought of saving alot(Metroid Prime) or had dreams in 3rd person(Pokemon and Prototype).

one time i was practicing for a guitar hero tournement and i would see the fret board and notes over peoples heads sometimes

Almost all day on Thursday I thought I was hearing the sound sniper bullets from Bad Company 2 make when they hit the ground beside you. Also, after playing GTA: SA for a few days, after I had been to a convenient store I was about to switch to my machine gun and shoot the cashier, except that one was out of habit, not the Tetris effect.

I had a similar dream but with BFBC2 and I was fighting with a controller.

When I had swine flu after playing to much Banjo Kazooie N&B, I imagined that everything was made out of building blocks and parts from the game. And I almost tried to "improve" my car by removing excess weight. Painkillers are fun.

I also get a HUD for about five seconds occasionally.

Anytime I hear the word shepherd, I think of two things: General Shepherd (the douchebag in Modern Warfare 2) or Commander Shepard (the douchebag/douche-baguette in Mass Effect 2).

There was a period during high school where I played a lot of Tetris. And after playing it I imagined a game in my head. Performing a lot of t-spin doubles can do that to you.

And sometimes I hear the achievement sound for the Xbox.

After playing a lot of RockBand 2, my vision was kind of scrolling down when I looked at walls. Just like the game, where the fret board is scrolling "towards" you, my vision was scrolling down. It was no biggie, just weird. It's a sign you have played too much RockBand.

Before I got my PS3 I used to hear the 360 achievement noise randomly. Hell, I still do, sometimes.

After a day or so of straight Halo 2 Legendary, me and my friends were seeing snipers on the rooftops... Bloody one shot kill aim bots.
