The only times I make an exception are when it's ridiculously hot, in which case I might use a fork until it cools down enough for me to eat it with my hands, or when I'm at some sort of fancy occasion and have to worry about my appearance. Both occasions are rare for me, though.

Edit: Damn you! Seriously! You have no idea how much I currently crave for pizza because of this post! Delicious cheesy slightly salt pizza with salami or seafood on top. Hmmmmmmmmmmmmm.

The real question is: What toppings do you have on your pizza? I needs me some bacon and beef, and I prefer to have extra cheese, pineapples, ham, and garlic, but that depends on who I'm eating with.
