I didn't vote for any of these games because the only one I've actually played is Dead Space. I would say the scariest game I've ever played is Metroid Fusion. You know if I say a GBA game is the scariest game I've ever played then I'm serious. You spend the entire game alone on a space station overrun by monsters. Your only contact is a heartless computer and you run around in the slowly deteriorating ship with the eeriest music I've ever heard trying to avoid the imminent death of running into the unstoppable SA-X. Close second, the island section of Resident Evil 4. The Regenerators scared the crap out of me.

Sure, but really? Resident Evil 4? Really? That's about as scary as house of the dead.
I thought it was frightening when I ran out of ammo in all my guns fighting a monster that would constantly regenerate its limbs and, much to my despair, can actually move faster without its legs.
It builds suspense when you can hear their raspy breathing somewhere down the hallway. You ready your shotgun only to discover that you're seriously depleted from fighting the previous regenerator. Besides those dogs with tentacles coming out of their faces, they're the only things that scared me in that game.
I get your point, dude. I don't scare easily (not being cocky here, I just don't, and wish I did) but I can tell you're just a person that enjoys the atmosphere in a game. I LOVED Metroid Fusion, and will agree with you that it can get a little creepy. Same for the RE:4 Regenerators.
Now, I can't think of any games that scare me nowadays, but back in the old days (when I was like 10) the Redeads from Zelda: Ocarina of Time used to scare the shit out of me. I actually couldn't finish the game for a while. I've grown up now to realize they they're just hump-zombies, but still, that's the best answer I can give for this thread.
