You need most characters loyal, and I sent mordin back with the crew, so he survived. Had big people like garrus and grunt holding the line during the end sequence. Had a good leader for the second team and tali as my tech expert. Worked well for me :D
To get the 3 deaths, im planning on sending tali in the vents and using a bad leader like mordin. Hopefully hell get himself killed in the process. This shoudl kill Tali. Then Jack isnt loyal to me so I can use her as my biotic specialist and that will kill her. Good times!..
Might be a while before I do that though because I just started playing FF6 again

In the evil ending of The Force Unleashed, I was sad when the pilot died, as I was when Starkiller died in the good ending (that is, until I realize there are about a thousand more of him).

Foehammer from Halo: Combat Evolved the poilet who gets hot down during the level The Maw she was fucking awesome and just about every minor character from halo.

Hinawa and Clous' deaths were done in such a way that it really is saddening, especially for Clous with the scenes that lead up to it.

I was almost in tears in the final battle.

Mine would have to be Kat in Reach. Boom, headshot. No fanfare, no epic scene, bugger all but Six shooting the Phantom with his pistol.

Poor SPARTAN III's. Got the short end of the stick in regards to armour, shields and augmentations.

I suppose a lot of people just didn't get attached to him, but still. And the entire premise of the story was sad. Look it up or play it, it's well-told despite relying on mostly gameplay and graphival changes in the character and music as opposed to cinematic storytelling.

OT: I would have to say that the death of the dad in Fallout 3 was shocking. I really did not expect it and was shaken when it happened.
