The hospital in Silent Hill 2 nearly made me crap my pants as soon as I stepped inside. I turned off the computer and didn't touch the damn game again until two weeks later.

Also, honorable mention goes to the Shalebridge Cradle from Thief: Deadly Shadows.

I called bullshit on that one

until the black one popped out, then I was like: really, guys?

then I went to the "sewers", man I can't even begin to tell you the amount of times I tripped over those fucking sleeping librarians, even with my night goggles on I tripped over them

well, that and stepping on the ribcages

edit: oh, I almost forgot, the cave in Ultima 7 (or the Forge of Virtue pack, don't remember properly), where them crazy fucks sacrificed people and shit

and that baby on the table thing, all dead and shit =P

of course, it was back in the day when it happened, so, yeah, I was, uh, "weaker" then

and let's not forget the UAC Station in Doom 64

I swear that's the most atmospheric game and overall scariest space station thing, of course, until System Shock 2 came out

T junctions in corridors, when you could have hostiles on either side.

Also, stairwells, where something could be waiting above and behind, as well as forwards and to the side.

This requires a realistic-y sorta game to really appreciate.

In particular, the maze of cubicles in the...was it the Red Library Offices level? Wandering round a maze, openings on all sides, a gallery above you and the enemy has gas masks.

Likewise, moving through book stacks in the Rainbow 6: Raven Shield expansion.

Im going to say the medical centre In Bioshock

it was just creepy , but it also remined me of the body horror what ADAM does to your face anf of caourse Steinman who wants to be picasso with peoples Faces

Nurse: "her nose looks so much better, you did a great job"
Steinman: "hmmm....you ever notice how her face...sags?"
Nurse: "excuse me doctor?
Steinman: "Nurse! scalpel!"
Nurse: "but..shes not booked in for a face lift!..doctor stop cutting! stop cutting!"

and of coarse

Steinmen: "UGLY!! THEYRE ALL UGLY!!!" and I think one of his subjects is actually alvie at the time

Also The unitology Centre in Dead Space 2 althouth that game isnt scary it was unerving to see what went on there

when I came across one of those "Brainwashing" curbicles with the lights and images and there was a dead woman there, I sort of get the feeling she didn't die from the necromorph outbreak

Yume nikki -the entire dream world freaks me out, but the dismembered body part room where you get the item that lets you go to the nexus is the worst I've encountered so far

While not exactly scarey, it sent a shiver up my spine and set me on edge. In the first dead space, after getting to the captains nest for the first time, your sent away again, down that elevator.... and as your going down out of no where theres this bloodcurdling womans scream, then silence... man I froze...

the school bus level from Prey.
first game to let me kill kids though, so that's a plus.

And the game world from Minecraft. You don't know terror until you go to take a peek out our door during your first night in a mysteriess land and get jumped by a gaint spider.

I would of gone with the water filled hallway, mostly because I was given the advise before hand to "stay out of the water."

One second I was returning home from a profitable expedition, heavy with iron, coal, redstone, and gold. The sun was high, so it was ok that I had no torches with me.

The next second I had one heart left, no armor, no torches, no sword, barely any food, I was roughly 30 blocks down a shaft into a cavern, and there were zombies.

I nearly pissed myself from terror.

Condemned: Criminal Origins is the only game that genuinely scares me. There is one room in particular, in the subway level. It's completely dark, and water covers the floor, and there are huge tanks off to one side of the room. In the middle is a raised platform that you can't jump up on. I don't know what it is, but this one room scares the shit out of me. I literally got the chills the last time I played through the game. When the pale and spiderlike subway freaks come I just go ballistic and whack the shit out of them. I find the female ones scare me more than the men.

Gary wasn't so bad for me, but the fact he has crap, you have assault rifle didn't entirely make it fair, although the whole "Gary?" "Gary!" "Garrrrreeeeeeee?" thing did weird me out a bit; he's everywhere! More Garys would have probably made it creepier for me.
The Fallout 3 Vaults weren't so bad for me, just curiosities (that hallucinating Vault was a bit of fail-scare.) That said, New Vegas' Vault 22 and 34 (the new ghoul sounds were just cringe-worthy) both creeped me out, although not as much as Dunwich;
nice to see I'm not the only massive coward when it comes to that place...
