Actually 2 mods have scared me the most and they are both for half life. Afraid of monsters and They hunger.

World of war craft scared me, it was the thought that I might suddenly get addicted and turn into a basement dweller,no offence intended to wow players(it was purely by accidental)

Though the atmosphere in uplink while not scary as such, it was an extremely tense game at points. Especially since there was no option to return your game to an earlier state(unless you cloned your save file) and could see your doom approaching via the means of a timer counting down.

Poison headcrabs + headphones = nightmares.

Other things that do not mix well with headphones and one's sleeping patterns:

Giant Radscorpions (legs clicking around your ears... nasty)

Tortured mutilated wall people in Dead Space... actually that whole game.

I was just going around with some wolfies when this sound glitch occured: and incredibly loud and horrible quality roar that sounds like... I can't compare it to anything. Probably the scariest thing I ever heard.

For me, the scariest game I've ever played was Fatal Frame 2. RE 4 and Dead Space were creepy, yes, but they both startled me more often than they scared me. Fatal Frame 2 both scared and creeped me out.

One of the scariest things for me was in RE4, when you're in control of Ashley. You're walking through that room of suited armour, thinking they'll jump out any second. Grab what you came for, think you're safe now.

WRONG. Holy cow, I've never run so much for my life ever before.

For me, I saw those suits of armor coming to life as soon as I saw the puzzle in the middle of the room. Just screamed of it.

Aliens vs. Predator!

Playing as a Marine was shit-your-pants scary. I've played all the supposedly scary games, but this is the only one that actually scared me. I actually just wanted to walk through a level and NOT encounter any aliens.

I am not a huge fan of scary games but the Labs in STALKER deserve a special mention. Poltergiests throwing any old crap at you, zombies and and bloodsuckers. The bloodsuckers turn invisable if your lucky if you hear them roar before they start tearing you apart.
