My favorite 'classic' scare has to be all of the original Aliens vs. Predator's Marine campaign on the Jaguar... at least up until getting ahold of the Smartgun. Slow room-by-room clearing, with just the sporatic tone of my scanner keeping me company, praying that I'll be able to get to an open area so that I don't fill a tight hallway with deadly pools of alien ichor.
More recently, I'd have to say Fallout 3 with Martigen's Mutant Mod installed has led to the most scary situations. The grandaddy of them all (which has been mentioned a few times now): Dunwich
Of course, I'd just updated MMM, and missed a few new features in the new build... namely reanimating feral ghouls (~35% chance that a "dead" feral will get back up after a random amount of time elapsed, and ONLY when out of line-of-sight).
I finished clearing the first room (truly more ferals than I could count) and I was starting the process of looting and scavenging the area when I turn around to find a trio of ghouls (a couple missing an arm... creepy) rising to attack. What followed was another several paniced minutes of RE-killing a score of ferals and then reducing the remains to chunks (body parts and casings from multiple calibres everywhere). Eventually, I learned that headshots and amputated legs will ensure only having to kill them once... But the sheer terror of that first encounter left emptying clips into anything and everything that had previously been among the living.

I think the first flood level in the original Halo where you watch the video tape left by the marines was my scariest game moment because I seriously was in 1st grade when I played that level and had no idea what was going on. Every time something happened I got closer to peeing myself.

To make matters worse a supernatural fog invaded the colony and makes the deceased spirits want to kill you. And you can hear them, but not see them unless they're within arms' reach. The only way to spot them in advance is with security cameras, and they are few and far between.
You die in the game when your heart sensor pretty much flatlines as you have a heart attack brought on by sheer terror.
Also, no soundtrack. The only things you hear are your breathing, your footsteps, your heart monitor, and the bloody voices of the DEAD!

Even though Comdemned wasn't that great of a game, it gave me the best scare I've ever had. In maybe the 6th level you are walking through a mall and manequins(spelling?) start moving around. Towards the end of the level you hit a dead end and turn around to find manequins lined up behind you, as you walk past and turn around you find them lined up differently, facing you now. Ho man! Scared the crap out of me! It was really late at night too.
